                         REPORT CARD QUESTIONNAIRE
The CFP Program uses this questionnaire to produce the CFP Report Card, which updates
NPS and other interested parties annually on the progress of the CFP Program.

1. PARK INFORMATION

   Date Completed:

2. EMISSION INVENTORY
If your park performed an emission inventory last year, please enter the results of that
inventory below.

   Inventory Year:

   Emission Inventory Results:
       Energy
       Transportation
       Waste
       Other Emission Sources
       Total
       Forest Management
If total forest management emissions were a net sink, please enter a negative value.

If new emission sources were included during the most recent inventory year, please list
them below.

   New Emission Sources

3. ACTION PLANNING
If your park revised its emission reduction goal during the most recent year, please list
the new goal below.

We pledge to reduce our baseline emissions of MTCE by percent by
2012.

Please complete Table 1 below by best-fitting the mitigation actions your park has taken
to the climate-friendly actions provided by management sector. If available, please also
provide an emission reduction associated with the mitigation action.
If your park has completed mitigation actions that do not fit into the actions provided in
Table 1, please enter them in Table 2. If available, please also provide emission
reductions associated with the mitigation action.
