3. Eligible Dwelling
An Eligible Dwelling is a self-contained dwelling located
at a distinct address and lawfully occupied (or suitable for
occupation) as a place of residence.

Eligible dwellings within a caravan or residential park
include the following:
   a caravan that is permanently located at a residential
    park.;
   mobile home that is permanently located at a
    residential park.; or
   cabin or like structure that is permanently located at
    a residential park.

Part D – Declaration
D1   You must agree to the following declaration in order to apply for the Scheme
     1.   I declare that the information provided in this application is true and correct and I understand that providing
          false or misleading information is a serious offence and carries penalties under the Criminal Code Act 1995.

     2.   I declare that I have read the Eligibility Criteria and that I satisfy them.

     3.   I acknowledge that I am required to make a household contribution of $200 (GST inclusive) to the installer and
          undertake to make this payment at the time of installation.

     4.   I acknowledge that I will be liable to pay a fee of up to $70.00 (GST inclusive) if I fail to attend an appointment
          with the contractor at the scheduled time.
     5.   I agree that the information provided in this application, including my personal information, may be
          transmitted to, and used by agencies of the Commonwealth of Australia or its agents and State or Territory
          governments or agencies, Government contracted antenna and satellite installers and broadcasting
          organisations for the purposes of the administration of this scheme or related schemes and programs and for
          regulation, enforcement, evaluation, and policy development.

     6.   I release the Commonwealth of Australia, on behalf of myself and all other people living at the dwelling, from
          all liability for any loss, damage, injury or cost incurred as a result of, or relating to, the provision of goods and
          services under the Satellite Subsidy Scheme.
