 Rebate form filled out by dealer before consumer files
 Homeowner will receive confirmation number
   If confirmation number isn’t provided, the claim was not filed
 Multiple systems per household allowed
   Separate forms per system must be used
 Check or direct deposit (approx. 4-6 weeks)
