1.    Entry fee

2. Registration
2.1 Registration must be made on the entry form, events with insufficient entries may have format altered or may
    be cancelled
2.2 Players may participant in any 2 category, Entry form submitted without payment shall be rejected
2.3 Players may be asked to produce Original IC and or Passport before or during the tournament, failing which
    the players will be automatically disqualified

3. Entry Form
3.1 Entry Form are obtainable from and must be sent /fax to the following collection centers :

3.2 Entries submitted through the respective Academy must be ranked for all events
3.3 Any entry form received after the closing date will not be entertained

4.    Payment
      Cash / cheque made payable to Looi Badminton Academy or to our bank a/c MBB 5641 1910 8765
      (A copy of the bank in slip/receipt must be faxed to 03 7983 0730 for verification purposes)

5.    Entry Closing date : 22 November 2010 @ 12 noon               Draw date : 26 November 2010

7.    Terms & conditions (General) :
7.1   The Championship schedule is subject to change at the discretion of the Referee.
7.2   The “ Rally Point” scoring system used is (30 points per match); Semi Final (21 points, best of 3 games)
7.3   The Organizer will restrict entries into all the championships events and reserve the right to have qualifying
      rounds in any event.
7.4   Alteration of competition schedule due unforeseen circumstances may arise and will be announced by the
      Referee.
7.5   Players must report to the referee 30 minutes before their scheduled time of play.
7.6   The decision of the Referee on points of law will be FINAL and no appeal will be entertained.
7.7   The organizers will not be responsible for any injury sustained, at any time, by any player or official arising
      from his participation in this Championship.

8.   Terms & Conditions (SS Star Award)

8.1 Most Promising Sportsman Award

        Awarded to a participant with the following criteria
            a. Boys age 15 and below
            b. Clinch the most medals
            c. Must be a district or state player
            d. Good attitude
            e. Recommendation by other coach

8.2 Most Promising Sportswoman Award

        Awarded to a participant with the following criteria
            a. Girls age 15 and below
            b. Clinch the most medals
            c. Must be a district or state player
            d. Good attitude
            e. Recommendation by other coach

8.3 Most Promising Coach

        Awarded to a participating coach with the following criteria
            a. whos’ players clinch the most medals
            b. Age 40 and below
            c. Full time coaching
            d. Produces players with good results ( eg: district, states, national ). The players must be trained
                by the said coach for a minimum of 2 years

8.4 Coach of the Year

        Awarded to a participating coach with the following criteria
            a. Must be in the industry for at least 5 years or more
            b. Proof of producing state or sport school player
            c. Producing national player an added advantage
            d. Respected and known by all in the industry
            e. Still aggressive in badminton

8.5 Sportsman of the Year

        Awarded to a well known community figure(s) with the following criteria
            a. Young, energetic and passionate in badminton
            b. Age 30 and below
            c. Proof of Great achievement in Badminton
            d. A role model for future generation
            e. Still aggressive in badminton

8.6 Models of the Year

        Awarded to a well known community figure(s) with the following criteria
            a. Young, energetic and passionate in badminton
            b. Age 30 and below
            c. Proof of Great achievement in Badminton
            d. A role model for future generation
            e. Still aggressive in badminton

8.7 Golden Academy Award

        Awarded to a participating Badminton Academy with the following criteria
            a. Proper academy setup (eg: training uniform, innovative, reliable)
            b. Minimum age of academy: 5 years
            c. A well known community academy

       9.    Terms And Conditions ( SS Badminton Talent Scholarships)
                The organizer (SS Team) will select 10 participants based on their performance deem fit by the SS team.
                Selected participant will be sponsored a full time Badminton training program provided by Looi
                 Badminton Academy (LooiBA)
                SS will sponsor all the cost of the Training Program
                Reasons for not being awarded a scholarship are not provided, as resources do not permit.
                Only participant with the following criteria will be included in the selection:
                      a. Age 9-15 years
                      b. Able to commit full time of 5 years
                                 i. Selected participants must stay at the provided accommodation
                                ii. LooiBA will assist in transferring of school to a nearby National School deem fit by the
                                    SS team.
                               iii. Selected participants must follow the schedule provided by LooiBA; included is a once a
                                    month break* where the selected participants are allowed to return home for a
                                    weekend.
                      c. Foods are not included in this Scholarship Program.
                All scholarship recipients should be in good mental and physical health and must advise the SS Team of
                 any medical conditions they may have. Medical report must be supplied by the participant upon
                 requested by the SS team.
                SS team and LooiBA will help source for additional funds for training equipments and tournament
                 expenses but does not guarantee the availability of the funds. Failing of securing additional funds will
                 resort to participants bearing the additional costs.
                SS and LooiBA reserves the right to investigate and possibly withdraw a person from the scholarship
                 programme, if there is adequate proven evidence of serious allegations of illegal activities discovered.
                SS and LooiBA reserve the right to make reasonable changes to this contract at any time and will advise
                 selected participants if this occurs, as and when required.

10.1.3       Triple Sharing @ RM230 nett per room per night with 3 Breakfast

10.2         Reservations
10.2.1       All room reservations must be submitted not later than 20th November 2010 to selena@looiba.com.my or
             fax at +603 7983 0730 on the prescribed form.
10.2.2       Reservation received after the date will be considered subject to room availability and may be offered at
             different prices.

10.3        Payment

10.4        General Information
10.4.1       Payment & Cancellation : Before 25th November 2010
10.4.2       Rates quoted are in nett, inclusive of 10% service charge & 5% government tax
10.4.3       Check-in time is at 2.00pm and check out time at 12noon.
10.4.4       All miscellaneous bills will be on guest personal account upon check-out
