Directions to Teachers:
1. Fee is $10 per entry.
2. Part: Indicate each student's approximate range as: HIGH (lighter sound, in-tune high register), LOW (heavier
   sound, more difficulty staying in tune on higher parts), or BOTH (in tune in all registers, capable of harmony)
3. Save and email form as an attachment to paula.vickers@stpsb.org.
   See separate instructions (at "Forms") if you are having trouble with this process.
4. Send a signed hard copy of this form with your check made out to District IX Vocal Account to Paula Vickers,
   Mandeville Middle School, 2525 Soult St., Mandeville, LA 70448.
