                 Treasure Coast Brownfields Revolving Loan Fund Program
                                   Cleanup Subgrants

         Grant Application Procedure:
         Treasure Coast Brownfields Revolving Loan Fund (TCBRLF) program subgrant
         applications and guidance will be provided upon request to interested public sector
         subgrant applicants. The applicant must submit a completed TCBRLF Subgrant
         Application to TCRPC to be considered for the program. The Project/Fund Manager
         will review the grant application and eligibility according to local, state and federal
         guidelines. If eligible, the program’s Loan Review Committee (LRC) will make the
         final decision to “approve”, “deny” or “approve contingent upon specific
         modifications” the funding request.

         Fees:
         The following non-refundable fee will be required with the completed application to
         the TCBRLF Grant Program. Checks must be made payable to Treasure Coast
         Regional Planning Council.

IV. ENVIRONMENTAL REMEDIATION

appropriate section(s) of the application and do not include with application. If you
are uncertain, please feel free to contact the program.

    All site assessment reports including all ASTM Phase I, Phase II or equivalent;
    Most recent Draft or Final Brownfield Site Rehabilitation Agreement (BSRA)
    with Florida DEP;
    Remedial Action Plan(s) (groundwater remediation plan, long-term groundwater
    monitoring plan, etc.)

The following documents are NOT part of the application, but will be
required upon the award and acceptance of a grant:

        The Final Remedial Action Plan which includes the following:

        1. An analysis of brownfields cleanup alternatives which will include
           information about the site and contamination issues (i.e., exposure
           pathways, identification of contaminant sources, etc.); cleanup
           standards; applicable laws; alternatives considered; and the
           proposed cleanup;

        2. The effectiveness, implementability, and the cost of the response
           proposed the evaluation will include an analysis of reasonable
           alternatives including no action;

            Quality Assurance Project Plan (QAPP) developed according to the
            Florida DEP approved Remedial Action Plan and approved by EPA;

            Public Involvement Plan (samples are available upon request).

V.    FINANCIAL INFORMATION

         profit or local government entity;
         If the year-end statements are over 90 days old, the most recent
         internally prepared financial statements;
         The most recent accounts payable and accounts receivable aging (if

VII. PROJECT FINANCING
To be eligible for grant funds, a project must show that sufficient financing is
not available from other sources without financing from the Treasure Coast
Brownfields Revolving Loan Fund Program. Please describe your efforts to
secure financing from other private and/or public sources and summarize the
reasons why participation in the TCBRLF program is necessary (attach pages
as necessary).

issuance of a loan or subgrant. All incurred legal fees for said representation
shall be the responsibility of the undersigned even if the financing shall fail
to close.

promptly inform the TCRPC of any changes in the proposed project which

assistance from the TCRPC constitutes agreement to include the TCRPC in
any public relations events or materials related to the project, and to
cooperate with and permit the TCRPC to publicize its involvement for
marketing and public relation purposes including, but not limited to: signage,
press releases, public events, and promotional materials.
