MM Medical Action, Medline and NOVAPLUS®/LSL Infusion Therapy Kits
     (MS80721, MS80722 and MS80723)
     Acute Care, Long-Term Care, Ambulatory Care
     Novation has awarded agreements for infusion therapy kits to Medical Action Industries
     (MS80721), Medline Industries (MS80722) and LSL Industries (MS80723; Supplier Diversity
     Program), effective May 1.
     LSL Industries’ proposal represented the best overall value, followed by Medical Action and
     Medline.
     Infusion therapy kits include:
              I.V. start/dressing change kits
              central line insertion/dressing change kits
              dialysis catheter insertion/dressing change kits
              PICC line insertion/dressing change kits
              implantable port insertion/dressing change kits
              custom infusion therapy kits
     The NOVAPLUS infusion therapy kits, awarded exclusively to LSL Industries, are new to
     this product category and will consist of limited codes. The NOVAPLUS codes are infusion
     kits containing a CHG-based antiseptic.

MM Furniture Portfolio
     All Classes of Trade
     Novation has awarded furniture contracts to 10 suppliers, effective May 1. Of the new
     suppliers, four are incumbents, seven are full-line suppliers, two were awarded through
     Novation’s Supplier Diversity Program and, for the first time, one awardee is a distributor
     rather than a manufacturer.
     The suppliers in the new Novation furniture portfolio are:
         •   Corporate Express Business Interiors (CE80141) is Novation’s first non-manufacturer to
             be awarded a contract for furniture. CEBI will distribute and report purchases for
             Novation-contracted suppliers Global, National (a division of Kimball) and Neutral
             Posture through those suppliers’ individual agreements. CEBI will distribute and
             directly report purchases for more than 200 other manufacturers including Allsteel,
             Hon, Cramer, Carolinas (OFS), Nemschoff and Wieland.
         •   Durfold Corp. (CE80142; Supplier Diversity Program) is a certified woman-owned
             company that specializes in sleeper chairs and sofas, patient and guest chairs, and
             lounge seating.
         •   Global Industries Inc. (CE80143) is a new Novation supplier that qualified as a full-line
             supplier in the areas of patient room furniture, bariatric seating, dining and cafeteria
             seating, ergonomic seating, executive and office seating, filing, fixed case goods,
             medical and tech seating, modular case goods, systems furniture, tables and task
             seating.
         •   Haworth Inc. (CE80144) is a new Novation supplier that qualified as a full-line
             supplier in the areas of dining and cafeteria seating, ergonomic seating, executive
             and office seating, filing, fixed case goods, lounge seating, medical and tech seating,
             modular case goods, systems furniture, tables and task seating.
         •   Herman Miller Inc. (CE80145) is an incumbent supplier that qualified as a full-line
             supplier in the areas of dining and cafeteria seating, ergonomic seating, executive

             and office seating, filing, fixed case goods, lounge seating, modular case goods,
             systems furniture, tables, task seating and clinical workstations. In February,
             Herman Miller finalized its acquisition of Brandrud. As a result, the Brandrud
             product line is included in this agreement.
         •   Kimball International (CE80146) includes both the Kimball Office and National lines.
             Kimball is a new Novation supplier that qualified as a full-line supplier in the areas
             of dining and cafeteria seating, ergonomic seating, executive and office seating,
             filing, fixed case goods, lounge seating, modular case goods, systems furniture,
             tables and task seating.
         •   KI/Krueger International Inc. (CE80147) is an incumbent supplier that was previously
             on contract as a full-line supplier. However, for this portfolio term, KI was not
             awarded a full-line agreement and will provide products only in the areas of patient
             room furniture, bariatric seating and pediatric seating.
         •   Neutral Posture Inc. (CE80148; Supplier Diversity Program) is an incumbent supplier
             that is nationally certified as a Women’s Business Enterprise by the Women’s
             Business Enterprise National Council. Neutral Posture provides ergonomic seating,
             chairs and accessories for office and lab areas.
         •   Steelcase (CE80149) is an incumbent supplier that qualified as a full-line supplier in
             the areas of patient room furniture, bariatric seating, dining and cafeteria seating,
             ergonomic seating, executive and office seating, filing, fixed case goods, lounge
             seating, medical and tech seating, modular case goods, systems furniture, tables, task
             seating and clinical workstations.
         •   Teknion (CE80151) is a new supplier that qualified as a full-line supplier in the areas
             of dining and cafeteria seating, ergonomic seating, executive and office seating,
             filing, fixed case goods, lounge seating, medical and tech seating, modular case
             goods, systems furniture, tables and task seating.

MM Varian Medical Systems RapidArc™ Education Symposia (CE60102)

     Varian Medical Systems is announcing a series of education symposia for its RapidArc
     radiotherapy technology for faster and more precise cancer treatments.
     These symposia offer clinicians the opportunity to learn more about a technology that makes
     it possible to deliver image-guided, intensity-modulated radiation therapy two to eight times
     faster than conventional IMRT or helical tomotherapy.

MM ASP Sterrad Large-Volume Incentive Program (CE50110)
     Acute Care, Ambulatory Care
     Through Dec. 28, Advanced Sterilization Products is offering a large-volume incentive
     program promotion for the purchase of two or more Sterrad ® 200 or Sterrad NX systems in a
     single order.
     Program guidelines are:
         •   The member must purchase two or more systems with a total value of at least
             $100,000 per the initial purchase order.
         •   This program cannot be combined with the multi-unit discount schedule or any
             other promotion except the EndoSuite, Right Start package and the Sterrad 100 NX
             system introductory price.
         •   The promotion tier discount listed in the table below is determined on the total
             amount of the order. The calculation is based on the member’s eligible Novation
             contract price, not the list price.
         •   For integrated delivery networks or large health care facilities that purchase systems
             with multiple purchase orders, in order to reap the full discount, the orders must be
             received in the customer care center on the
             same day.
         •   IDN facilities may aggregate purchases under this promotion.
         •   All orders must be booked and shipped by Dec. 28.

MM ASP Cidex® Price Reduction Opportunity (MS60020)
     Acute Care, Long-Term Care, Ambulatory Care
     Effective March 1, agreement MS60020 has been amended to allow members to achieve an
     additional discount on the one-gallon size of Cidex OPA solution, case product code 20390.
     Novation and ASP have added a tier “A” for individual members only and a tier “B” for
     multihospital systems/integrated delivery networks.
         •   To qualify for the tier A price, the acute care or surgery center member must have
             purchased at least $20,000 of product code 20390 in the previous 12 months and
             maintain annual purchases of product code 20390 of at least $15,000.
         •   To qualify for the tier B price for acute care facilities or surgery centers requires a
             system/IDN to have at least $40,000 of product code 20390 purchases in the previous
             12 months and maintain annual purchases of product code 20390 of at least $30,000.
     The tier A and tier B price for Cidex OPA product code is $96.66 per case. Members must

     sign a new, revised letter of participation to receive these reduced prices.

     The MIST Therapy system is a painless, noncontact ultrasound device that heals wounds
     without the traditional ultrasound requirement of direct contact with the skin.

FS U.S. Foodservice Distribution Agreement Enhancements (FD90033)

    In conjunction with the recent extension of agreement FD90033 with U.S. Foodservice, the
    following two enhancements have been made to the agreement:

    participating members that can pay invoices within 16-20 days:

        •   The Authorized Distributor and participating member will identify and target the
            volume of new products that the member will purchase under the growth program.
        •   Upon execution of the growth program agreement and for a period of 90 days
            thereafter, the Authorized Distributor will implement the new applicable average
            order size incentive.
        •   At the end of the 90 days, the Authorized Distributor and the participating member
            will review actual purchasing activity and adjust the average order size incentive as
            applicable to the actual level of purchases made during the growth period.
