NOTES:

ELECTRONIC SUBMISSION OF PRICE SCHEDULE: This Price Schedule must be submitted in both electronic and hard copy. The
electronic copy of the Price Schedule must be submitted in its original Microsoft Excel Format on either a CD-ROM or USB Flash Drive.
The structure of the schedule is protected and shall not be modified in any way. Modified schedules may be deemed non-conforming
to the RFP. In the event of a discrepancy between the hard copy and the Microsoft Excel file on CD-ROM or Flash Drive, the hard
copy will take precedence.
BASIS OF AWARD: The Authority will award a contract to the conforming, responsible offeror with the lowest total price for Items
017113-5.1 through 322504-1.1.
PROPOSED PRICE SHALL NOT INCLUDE COSTS FOR INSURANCE PREMIUMS AND BONDS FOR OFFEROR OR
SUBCONTRACTORS OF ANY TIER.
NOTICE: The total cost of bond premiums for coverage required by this contract for the Offeror and Subcontractor(s) shall be
reimbursed by the Authority based on actual costs incurred, providing such costs are reasonable and customary based on industry
standards. Offerors should refer to Solicitation Provision 19 for bond reimbursement requirements for this contract. Such costs shall
not be included in the Offeror's proposal price.

NOTICE: This contract is covered by the Owner Controlled Wrap-up Insurance Program (OCWIP) and participation by the contractor
and subcontractor is required. OCWIP coverage, as described in Contract Provision 20 is provided by the Authority at no cost to the
contractor, except as described in 20.B which, if required, will be reimbursed to the contractor.
COMMENCEMENT, PROSECUTION AND COMPLETION OF WORK: The Contractor shall be required to (a) commence work under
this contract within 10 calendar days after the date the Contractor receives Notice to Proceed; (b) prosecute the work diligently; (c)
complete all work in all phases of the project within 504 consecutive calendar days from NTP; (d) complete Milestone #1 within 90
calendar days of NTP; (e) complete Milestone #2A within 100 consecutive calendar days of NTP; (f) complete Milestone #2B within 135
consecutive calendar days of NTP; (g) complete Milestone #2C within 210 consecutive calendar days of NTP; (h) complete Milestone
#2D within 285 consecutive calendar days of NTP; (i) complete Milestone #2 within 375 consecutive calendar days of NTP; (j) complete
Milestone #3 on April 5, 2012; (k) complete Milestone #4 within 504 consecutive calendar days of NTP; The time stated for completion
