1.      PROJECT INCREMENTALITY:

         What are the specific objective and benefits of the proposed project?
         Incrementality of proposed project (see “Incrementality” definition below)
         Summarize jobs to be created by proposed project (person weeks)
         A letter from MNR District Manager confirming Incrementality and support for the project must be attached

“Incrementality” means:
     Seedling production, site preparation and planting in areas of natural disturbance for which the SFL holder had no
       responsibility for renewal.
     Seedling production, site preparation and planting in areas previously identified for natural or basic renewal activities.
2.      BRIEF DESCRIPTION OF THE PROPOSED WORK
         How many seedlings will be grown and which species? How many hectares will be site prepared and/or planted?
         Is the proposed work consistent with the FMP and SGRs? If not, what action is being taken to ensure that it does
           conform?
         If this project is an enhancement from a natural or basic renewal prescription, please describe the originally proposed
           prescription.
         Confirm that any treated areas will be considered for tending if required. These areas could be eligible for future
           funding support from Forestry Futures Trust.
         If required, explain the tree “swapping” process to be followed
         Please provide a map that shows the geographic location of the project (so that an auditor could locate the site – i.e.,
           scale, north arrow, label with lake/river names, direction to or location of closest town, highway, township, names).
           Use appropriate scale to effectively show the distribution of treatment sites in the most efficient and effective way.

4.       REIMBURSEMENT SCHEDULE
          Reimbursement may be sought through interim and final invoices. Indicate the schedule of submission of Invoices for
            Reimbursement and accompanying status report (month and fiscal year). What amount of the total proposed funding
            will be sought in each invoice?
          The final invoice must be received within 30 days of project completion and must be not less than twenty percent
            (20%) of proposed funding.
          Standard payment practices for seedling production will apply.
          Interim Project Work Report is due March 31, 2010and the Final Project Work Report within 30 days of project
            completion.

evaluate the project, audit the project or to prepare reports or provide information as may be requested under the Crown Forest
Sustainability Act.
