AGREEMENT REGARDING RELATIONSHIP
On my own initiative, I hereby disclose that I am involved in a voluntary, consensual dating relationship with another employee of COMPANY (“COMPANY”). I acknowledge that prior to signing this Agreement Regarding Relationship (“Agreement”) I have reviewed the COMPANY’S sexual harassment policies. I understand and will abide by such policies, and agree that I will immediately report any suspected harassment or other violations of the policies, involving my relationship with ________________________ (“Relationship”), or otherwise. In particular, I agree that I will immediately inform either my supervisor, manager, or COMPANY’S Vice-President of Legal of any problems including, but not limited to, pressure, intimidation or harassing or hostile behavior of any kind, that may develop in the future in connection with the Relationship. I also state that no such problems presently exist. I agree that I will behave in a proper, professional manner in the workplace, that I will not engage in offensive workplace behavior, and that the Relationship will not negatively impact workplace productivity or responsibilities. I further agree that where the Relationship concerns a supervisor-subordinate working relationship, at least one party to the Relationship will transfer to another department or work group. I understand that this Agreement in no way alters the at-will status of my employment relationship with the COMPANY, and does not alter or replace any contract that may exist regarding my employment with the COMPANY. By signing below, I agree to abide by the provisions of this Agreement and acknowledge that I am signing this Agreement voluntarily and without duress.
