Check List and Information Request
o Name, address, telephone number, and email address of the church
o Name, address, telephone number, and email address of the person with whom we
  should correspond
o Copy of existing Articles of Incorporation, and bylaws, if any
o Original signed Articles of Restatement (signed by Clerk of Ministry and Counsel or
  Chair of the Elder Board OR Pastor, and Recording Clerk)
o Completed Nevada Restated Articles of Incorporation Document.
o Check made payable to the Nevada Secretary of State for $140
o Check made payable to Runquist & Zybach LLP for $75.00 to cover filing of restated
  articles and arranging for publication; if minutes are desired add an additional $75.
  Articles will have to be published after being filed; there will be a separate charge for
  publication.

Checklist for churches wanting minutes to be drafted
Please include the following information to the attorney’s office.
o Name and Address of the church (if the address of the church meeting place is
   different from the mailing address, please give both addresses)
o Telephone numbers of the church (office and fax)
o E-mail address and web address of the church, if any
o Names of each Ministry and Counsel member (Elders/Board of Elders)
o Names of all officers including:
        o Clerk of Ministry and Counsel or Chair of the Elder Board
        o Clerk of the Meeting
        o Recording Clerk
        o Treasurer
o What bank(s) you will have accounts with, and type of accounts (e.g. checking,
   savings, money market)
