Catholic Charities Diocese of Stockton announces the expansion of our San Joaquin
Senior Services with the addition of the Multipurpose Senior Services Program
(MSSP). Through the MSSP program Catholic Charities will provide
comprehensive case management for low-income, frail seniors with the goal of
preventing premature institutional placement of elderly clients and coordinating
services which promote safety and independence. As an MSSP site Catholic
Charities is contracted by the Department of Aging to provide social and health care
management of frail seniors.
Compensation based on qualifications and experience and includes medical
insurance, health, vision, dental, and paid time off. Individuals with compassion for

The anticipated starting date is June 1, 2009. The following positions are now open:
o Supervising Care Manager (35 hours) – This position requires a Masters degree
in social work plus two years experience working directly with the elderly. One
year of supervisory experience preferred. Primary responsibilities include ongoing
supervision of care management staff, and implementation of comprehensive care
management for the frail elderly. This position combines both administrative
duties and clinical supervision responsibilities. 1 open position.
o Nurse Care Manager (40 hours) - This position requires a current valid license
as a registered nurse in the State of California, plus 1-2 years work experience
with the elderly. Possession of a CA State Public Health Nurse certificate is
preferred. Responsibilities include conducting in-depth assessments/reassessments
of clients, certifying level of care determinations, coordinating with physicians
and other health professionals to implement care plans. 1 open position.
o Social Work Care Manager (35 hours) - Qualifications include a Master’s
degree in social work, nursing, psychology, counseling, rehabilitation,
gerontology or sociology plus 1 year experience working directly with the elderly
or Bachelors degree in one of the above field plus 2 years experience working
with the elderly. Responsibilities include providing care management, evaluating
and identifying functional and health limitations that impede independent living,
identifying support systems, as well as client referrals, and case recording and
reporting. 3 open positions.
o Care Manager Aide (35 hours) – This position requires a Bachelor’s degree in a
human services discipline, or two years of direct service experience with the
elderly. Excellent client service skills needed to complete duties such as client
intakes, assist in client contacts and monitoring, as well as participating in client
case conferences and care plan development. 1 open position.
o Program Office Assistant (40 hours)- Applicants need to be proficient in data
entry, Word, Excel, and office management skills. A minimum of two years work
experience is preferable. 1 open position.
