I am aware of those risks, and I am voluntarily participating in this activity with knowledge of the risks involved. I agree to accept any and all such risks of injury, death and/or property damage. I agree to the terms of this waiver, release, covenant not to sue and indemnity agreement as set forth herein. In case of injury or illness, I give my consent to emergency transportation and the administration of first aid, medical and/or dental treatment. I accept responsibility for the payment of any emergency transportation, treatment expenses and any related or subsequent medical and/or dental bills. I acknowledge that Haynes Pains Athletic Services, LLC (Hereinafter “HPAS”) has not purchased and does not provide any medical or accident insurance to cover such expenses. Any such insurance is my responsibility. I waive, release, absolve indemnify and agree to hold harmless HPAS, it‛s members, officers, directors, employees, volunteers, agents or any other representative of HPAS against any and all causes of action, claims, demands, losses, expenses, ability. In addition, I understand that my conduct, if deemed inconsistent with the rules of good sportsmanship and fair play, or the HPAS Rules and Regulations may result in my expulsion from this and other programs. I agree that if registered as a member team, the HPAS Sports staff may audit my players‛ member status at any time. I have fully read this document, understand it‛s meaning and legal impact thereof, have had the opportunity to have my attorney view this document and explain it to me prior to signing my name below. I voluntarily, of my own free will and without coercion sign this waiver, release, covenant not to sue and indemnity agreement. I have fully read the parent/player code of conduct, understand its meaning and agree to abide by all things covered. Any photographs taken while participating in any program, recreational activity, or event are the property of the Haynes Pains Athletic Services, LLC and may be used at their discretion. Parent Name: ________________________________________ Parent‛s Signature: ____________________________________ Sport: ______________________________________________ Date: ___________ PARENT/PLAYER CODE OF CONDUCT YOUTH SPORTS All sport programs offered by Highlands Ranch Community Association are based on fun and participation. We realize there is a competitive situation when two teams are playing each other, BUT winning is not our primary goal. We expect coaches, spectators and team members to participate with this philosophy in mind. HRCA has a zero tolerance policy for coach, player and spectator misconduct. Youth & Adult Sports Recreational Mission To develop positive sportsmanship skills and athletic ability in a positive, safe, and fun environment. Players, coaches, parents, spectators and staff all play an important role in meeting our mission. The Code of Conduct for Youth and Adult Sports Players, coaches and spectators are expected to support league staff and officials, treat the facilities with respect and demonstrate good sportsmanship at all times. Zero Tolerance Statement for All Sports Verbal and physical abuse, as well as abuse of facilities will not be tolerated. Infractions may be grounds for a suspension period or expulsion from future HRCA league participation. The zero tolerance policy applies to players, coaches, and spectators. All parents, spectators, & players are required to follow the following conduct: • Parents and spectators are expected to encourage their team and their opponents. • Parents, spectators and players are expected to support our league officials and understand that officiating is not an easy task. Officials are guaranteed to make errors just as everyone else does in the course of a game. • All players and spectators are expected to follow the facility guidelines, and be respectful of the facility at all times. No food or drink is permitted in the gym. Players may ONLY have closed capped drinks. • Parents are expected (and required) to treat all players, coaches, officials and HRCA representatives with respect. • Negative comments towards players, coaches, officials or HRCA representatives will result in the parent being asked to leave the facility. Failure to comply will result in an automatic forfeit for their team. • Aggressive behavior towards participants or other spectators will not be tolerated. Behavior of this type will result in being escorted from the facility by a member of the Douglas County Sheriffs Office. NO EXCEPTIONS WILL BE MADE TO THIS RULE. • Parents and spectators must stay in the designated area. Game officials will have the authority to call a forfeit if a parent refuses to comply with game officials directions regarding parents in off limits areas. • If a parent, spectator or player commits any of the following acts they will be ejected from the game and facility. If a head coach is ejected and a league approved assistant coach is not available to coach the remainder of the game it will be considered a forfeit for the offending team. • Disrespectfully addressing a league or facility staff person. • Using profane or inappropriate language or obscene gestures to a league or facility staff person. • Physically touching, pushing or hitting a league or facility staff person. Game officials have full discretion to interpret and apply the rules. Game officials‛ decisions will be final, and results will not be subject to appeal.
