I am making a commitment to a full program of study chosen from the Academy courses in the attached course list. 2. To be placed in an Academy internship, I must: Maintain a “B” average in my Academy courses; Have completed four credits of Academy coursework; Achieve an excellent record of attendance and punctuality; Attend an Internship Orientation Meeting; Prepare my Interview Portfolio (resume, cover letter, work samples and attendance record) and get two teachers or former employers to complete and submit Confidential Evaluations on me; Participate in at least one preliminary, Advisory Board panel interview; Provide my Social Security Number, working papers, proof of citizenship or immigration status, and fingerprints, if necessary; Be interviewed by my potential employer; and Pass an honesty test and/or medical examination, if applicable. 3. I understand that my internship: is a learning experience that will consist of 180 hours of paid work, in a position related to my program of study; will include coursework and assignments to be completed electronically (over e- campus); is generally completed by working full-time during the summer following my junior year; is worth one credit toward graduation; and will be identified through the efforts of the students and their parents, teachers and the NAF-PUSD Advisory Board. 4. I understand that any history of drug use or criminal conviction will jeopardize my internship. 5. By signing this memorandum, my legal guardian gives me permission to take trips with my class, report for job interviews, and travel to work unaccompanied for my Academy internship. 6. I must report for Academy activities, job interviews and work dressed appropriately in business attire. 7. Once placed in my internship, I am responsible for whatever tasks I am assigned by my supervisor. If I question the appropriateness of a particular assignment, I will contact the Internship Coordinator for advice. 8. I will complete, at my expense, at least three credit hours of approved college coursework on a college campus, or the equivalent, before high school graduation. 9. I understand that there is no guarantee of a job after graduation or following the prescribed dates of my internship experience. 10. If I have any questions or problems with regard to any aspect of the NAF-PUSD program I have chosen, I will contact an Academy instructor at my school; the Internship Coordinator; or the Academy Director, Gay Evangelista at 623-412-5334. In conclusion, I understand that I will receive a “Certificate of Program Studies” from the Academy program, if I have successfully completed all of the requirements listed and maintained a “B” average in all Academy courses. I also understand that I must maintain an overall “B” Grade Point Average (GPA) to receive special recognition at graduation. I understand that by being accepted into this program, I am committing myself to work during the summer following my junior year, unless other arrangements are made.
