2. GENERAL CONDITIONS (City of St. George)

In Section 15 TIME FOR COMPLETION AND LIQUIDATED DAMAGES, Subsection 15.5,
Construction Phasing and Liquidated Damages, change Subsection 15.5 to read as follows:
“15.5 Construction Phasing and Liquidated Damages. Construction sequence shall include
the following phases:
Phase 1: Phase 1 requires the over excavation of the Terminal and ARFF building pad
areas to the limits indicated on Sheet Numbers GR-39, GR-40, and DT-10 of the bid
drawings. Phase 1 also requires building pad preparation for the Terminal Building including
Crushed Structural Fill - Building Pads and Select Structural Fill to the limits indicated in
Detail 3 on Sheet Number DT-10, and on GR-39, and GR-40.
Phase 2: Phase 2 requires the completion of the GA Access Road from STA 10+50 to STA
18+00 as described on Drawing Sheet Numbers PP-43 and GR-60, and as described in
Technical Specification Section P-152 - Excavation and Embankment.
Phase 3: Phase 3 requires completion of the building pad preparation including Crushed
Structural Fill - Building Pads and Select Structural Fill to the limits indicated in Detail 3 on

Documents. If the Alternate Bid is not awarded, Phase 4 is Completion of the Project.

Plus Alternate is not awarded, Phase 5 shall become null and void.

3. GENERAL PROVISIONS (FAA)
ITEM NO. 6
In Section 80 PROSECUTION AND PROGRESS, Subsection 80-08 FAILURE TO
COMPLETE ON TIME, change Subsection 80-08 to read as follows:
“80-08 FAILURE TO COMPLETE ON TIME. For each calendar day, as specified in the
contract, that any work remains uncompleted after the contract time (including all extensions
and adjustments as provided in the subsection titled DETERMINATION AND EXTENSION
OF CONTRACT TIME of this Section) the sums specified in the contract agreement and
contract proposal as liquidated damages will be deducted from any money due or to become
due the Contractor or his/her surety. Such deducted sums shall not be deducted as a
penalty but shall be considered as liquidation of a reasonable portion of damages including
but not limited to additional engineering services that will be incurred by the Owner should
the Contractor fail to complete the work in the time provided in his/her contract.
Phase 1: Phase 1 requires the over excavation of the Terminal and ARFF building pad
areas to the limits indicated on Sheet Numbers GR-39, GR-40, and DT-10 of the bid
drawings. Phase 1 also requires building pad preparation for the Terminal Building including

Phase 2: Phase 2 requires the completion of the GA Access Road from STA 10+50 to STA
18+00 as described on Drawing Sheet Numbers PP-43 and GR-60, and as described in
Technical Specification Section P-152 - Excavation and Embankment.

In Section 9 ENVIRONMENTAL REGULATIONS AND PROTECTION, Subsection 9.15
GENERAL CONSTRUCTION STORM WATER PERMIT, change the last sentence in Item
No. 3 Projects That Disturb Greater than Five Acres, to read as follows:
“The Engineer has determined that this project will disturb more than 5 acres and that
requirement 3 described above applies for this project.”

5. TECHNICAL SPECIFICATIONS – VOLUME 2
ITEM NO. 8
In Item G-100 MOBILIZATION/DEMOBILIZATION, Section G-100-2.1, add Paragraph j. as
follows:
“j. Final cleanup and inspection.”
ITEM NO. 9
Add G-100 Mobilization/Demobilization, Section G-100-2.1 Paragraph l. as follows:
l. Construction Water Pond and Appurtenances. Contractor shall construct a pond and
appurtenances for use in storing water used for construction purposes. The Contractor will be
required to take all of the construction water it uses from this pond. The pond shall be at least 30 feet
wide by 30 feet long, measured along the bottom, by 6 feet deep. The pond shall be lined with an
HDPE welded seam geomembrane material of at lease 60 mil thickness.
The Contractor will be required to construct a pipeline feed from the nearest available City water
source – which is near the construction access point to the site. The pipeline shall be buried where
necessary so as not to create a traffic hazard. The Contractor shall provide backflow preventer(s) and
meter(s) of the sizes and nature required by the City’s utility department.
The Contractor shall leave the pond, liner, all pipes, valves, meter, backflow preventer, and all other
appurtenance required for a fully function construction water delivery and storage system. The
Contractor may retain possession of equipment used to take water from the pond and load it into
equipment it uses to transporting water around the construction site.
The Contractor is required to repair or replace any leaks to the liner and/or water delivery system for a
period of three years after substantial completion of the work. The Contractor shall also be
responsible for repairing or replacing any malfunctioning equipment. Repair work shall be completed
within 48 hours of notification by the Owner. The Contractor will not be responsible for repair or
replacement of materials or equipment that was caused by the negligent and/or willful destructive acts
of others.
The Contractor shall submit shop drawing for Engineer review.
ITEM NO. 10
Add G-100 Mobilization/Demobilization, Section G-100-3.3 Paragraph l. as follows:
100-3.3 Payment for Construction Water Pond and Appurtenances will be made at the unit lump sum
bid price which shall be full compensation for all work associated with constructing and maintaining
the pond and appurtenances. A single payment will be made when the pond is fully operational and
accepted by Engineer

In Item P-152 EXCAVATION AND EMBANKMENT, Section P-152-1.2, delete Paragraph b.
in its entirety and replace with Paragraph b. in its entirety as follows:
“b. Rock Blasting. Blasting may be required for solid rock in ledges, in bedded
deposits, in unstratified masses, conglomerate deposits, and cemented caliche deposits
which have a Rippability Index greater than 1,000 as determined by ASTM STP 984 and are
so firmly cemented they cannot be removed by using rippers on a properly operating Cat D-
10 bulldozer (operating mass of 171,675 pounds at 700 HP; flywheel power of 522 kW). The
presence of isolated boulders or rock fragments larger than one (1) cubic yard will not in
itself be sufficient cause to change the classification of the surrounding material. In variable
deposits, “Rock Blasting” shall be deemed required for material where a Cat D10R with a
single shank has a production rate less than 250 bank cubic yards per hour. The effort
associated with blasting, where deemed required by this paragraph, will be compensated for
in the “Rock Blasting” Pay Item.”
ITEM NO. 13
In Item P-152 EXCAVATION AND EMBANKMENT, Section P-152-2.2, delete Paragraph b.
in its entirety and replace with Paragraph b. in its entirety as follows:
“b. Undercutting. Rock, shale, hardpan, loose rock, boulders, or other material
unsatisfactory for safety areas, subgrades, roads, shoulders, or any areas intended for
seeding shall be excavated to a minimum depth of 12 inches (300 mm), or to the depth
specified by the Engineer, below the subgrade. Muck, peat, matted roots, or other yielding
material, unsatisfactory for subgrade foundation, shall be removed to the depth specified.
Unsuitable Material shall be disposed of at locations shown on the plans. This excavated
material shall be paid for at the contract unit price per cubic yard for “Unclassified
Excavation”. The excavated area shall be refilled with Suitable Material obtained from the
grading operations and compacted to specified densities. The necessary refilling will
constitute a part of the embankment. Where rock cuts are made and refilled with Suitable
Material, any pockets created in the rock surface shall be drained in accordance with the
details shown on the plans.”
ITEM NO. 14
In Item P-152 EXCAVATION AND EMBANKMENT, Section P-152-2.2.e, delete the first
paragraph in its entirety and replace with the first paragraph in its entirety as follows:
“e. Compaction Requirements. The bottom of excavation, and/or top of existing soils
prior to fill placement, shall be compacted to a density of not less than 90 percent for fine-
grained soils (material with 50 percent or greater passing the No. 200 sieve) or 95 percent
for granular soils (material with less than 50 percent passing the No. 200 sieve) of the
maximum density as determined by ASTM D 1557. The material to be compacted shall be
+2 percent of optimum moisture content before being rolled to obtain the prescribed
compaction (except for expansive soils).”

In Item P-152 EXCAVATION AND EMBANKMENT, Section P-152-2.2.e, delete the third
paragraph in its entirety and replace with the third paragraph in its entirety as follows:
“The density of soils in the bottom of the excavation, and/or the upper surface of soils prior to
fill placement, shall be tested at least once per 2,500 square yards (2,100 square meters).
Laboratory moisture and density testing will be performed on material samples taken before
compaction. Laboratory moisture and density determination will be performed according to
ASTM D 1557.”

In Item P-152 EXCAVATION AND EMBANKMENT, Section P-152-2.6, delete the first
sentence of the sixth paragraph in its entirety and replace with the first sentence of the sixth
paragraph in its entirety as follows:
“The material in the layer shall be +2 percent of optimum moisture content before rolling to
obtain the prescribed compaction; except the claystone/mudstone material shall be +2
percent to +4 percent of optimum moisture content.”
ITEM NO. 17
In Item P-152 EXCAVATION AND EMBANKMENT, Section P-152-2.6.1, delete Paragraph
a. in its entirety and replace with Paragraph a. in its entirety as follows:
“a. All embankment material shall be granular well-graded, 4-inch minus material placed
in lifts not to exceed 8 inches in loose depth.”

In Item P-152 EXCAVATION AND EMBANKMENT, Section P-152-2.7, delete the last
paragraph in its entirety and replace with the last paragraph in its entirety as follows:
“No Crushed Structural Fill shall be placed on the grade until such grade has been approved
by the Engineer.”
ITEM NO. 19
In Item P-152 EXCAVATION AND EMBANKMENT, Section P-152-2.9, delete Paragraph 2.9
in its entirety and replace with Paragraph 2.9 in its entirety as follows:
“152-2.9 Tolerances. The top of the finish grade shall be of such smoothness that, when
tested with a 16-foot (4.8 m) straightedge applied parallel and at right angles to the
centerline, it shall not show any deviation in excess of 1/2-inch (12 mm), or shall not be more
than 0.10-foot (0.03 m) from true grade as established by grade hubs or pins. Additionally,
the average elevation of all actual elevations on 50 foot intervals shall not vary from the
design elevations shown on the drawings by more than 0.05-foot. Any deviation in excess of
these amounts shall be corrected by loosening, adding, or removing materials; reshaping;
and recompacting by sprinkling and rolling.”

4. It is understood that the schedule of minimum wage rates, as established by the
Secretary of Labor and included in the Specifications, are to govern on this project, and
the undersigned certifies that s/he has examined this schedule of wage rates and that
the prices bid are based on such established wage rates.
5. The undersigned prime contractor, if not a certified DBE, hereby assures that
he/she will make sufficient and reasonable efforts to meet the DBE goals, that he/she
will subcontract 4.5% percent of the dollar value of the prime contract to DBE firms, and
that he/she will include the DBE clauses required by the sponsor's DBE Program in all
subcontracts which offer subcontracting opportunities. The undersigned will complete
and submit with the bid the attached DBE Participation Form. If unable to meet the
project goal, the undersigned shall submit a demonstration of good faith effort in
accordance with the Federal Conditions of the Contract Documents.
6. The undersigned agrees upon written notice of the acceptance of this bid, that
within thirty (30) calendar days after the award, that s/he will execute the contract in
accordance with the bid as accepted and give contract (Performance and Payment)
bonds on attached forms within ten (10) calendar days after the prescribed forms are
presented for signature.
7. The undersigned further agrees that if awarded the contract, s/he will commence
the work within ten (10) calendar days after the receipt of a notice to proceed and that
s/he will complete the work within 120 Calendar days if only the Base Bid is awarded or
150 Calendar Days if Base Bid Plus Alternate is awarded, after notice to proceed. An
extension of time may be allowed when extra or additional work is ordered by the
Engineer. Liquidated damages will be charged for each calendar day that exceeds the
number of Calendar days allowed for each phase as provided in Section 15 of the

THIS AGREEMENT, made and entered into this ____________ day of
_____________________, 20______, by and between City of St. George, Party of the
First Part, hereinafter referred to as the “Owner”, and

Second Part, hereinafter referred to as the “Contractor,” for the construction of airport
improvement including and
other incidental work at the St. George Replacement Airport.
WITNESSETH:
ARTICLE 1. It is hereby mutually agreed that for and in consideration of the payments
as provided for herein to the Contractor by the Owner, the Contractor shall furnish all
labor, equipment, and material and shall perform all work necessary to complete the
improvements in a good and substantial manner, ready for use, and in strict accordance
with this Contract, a copy of which is filed pursuant to law in the office of the legal
representative of the Owner, and the Contract Documents defined in Article 2.
ARTICLE 2. It is hereby further agreed that in consideration of the faithful performance
of the work by the Contractor, the Owner shall pay the Contractor the compensation due
him/her by reason of faithful performance of the work, at stated intervals and in the
amount certified by the Engineer, in accordance with the provisions of this Contract.
ARTICLE 3. It is hereby further agreed that, at the completion of the work and its
acceptance by the Owner, all sums due the Contractor by reason of his/her faithful
performance of the work, taking into consideration additions to or deductions from the
Contract price by reason of alterations or modifications of the original Contract or by
reason of “Extra Work” authorized under this Contract, will be paid the Contractor by the
Owner after completion and acceptance.
ARTICLE 4. It is hereby further agreed that any references herein to the “Contract” shall

ARTICLE 6. The Contractor agrees to commence work within ten (10) calendar days
after the receipt of a notice to proceed, and the Contractor further agrees to complete

Days if the Base Bid Plus Alternate is awarded, after notice to proceed. Extensions of
the Contract time may only be permitted by the execution of a formal modification to the

Question No. 1: Is your export area going to be structural fill or just placed in there as a
dump and run?
Answer: The compaction requirements are contained in the bid documents- Including
but not limited to Specification P-152 and Drawing DT-11. Refer to Addendum No. 4
including but not limited to Specification P-152-2.2.e.
Question No. 2: On page GP-35 it says that the completion of project 90 calendar days
from NTP, which is in conflict with a couple of other locations on your contract document.
Most everything else has 120 days.
Answer: Refer to Addendum No. 4 including but not limited to 2. GENERAL
CONDITIONS (St. George) Section 15.
Question No. 3: 9.8.12 #3 Fugitive Emissions States: “ at the completion of any project
that produces exposed surfaces that have the potential to produce fugitive dust, the
contractor shall take measures to prevent such fugitive dust from being produced. Such
measures shall be effective for the time period that is specified in the contract. Should
fugitive dust develop within this time the contractor shall, at his/her own expense return
to the site and re-stabilize the material. The choice of stabilization material shall be the
contractors, unless the owner specifically specifies such material” What is the contract
time for this item? And what is the material specified? Where does this item of work get
paid for?
Answer: For stabilization requirements refer to the City of St. George Air Quality
Ordinance No. 2007-12-005, Section 4-9-8 (6) a. & b. Refer to Addendum No. 3 and
Addendum No. 4. There is no separate line item for this work.
Question No. 4: Is there a different schedule for base bid vs. alternate bid or is just 120
days no matter what you pick up?
Answer: Yes, see Addendum No. 4 including but not limited to 2. GENERAL
CONDITIONS (St. George) Section 15.
Question No. 5: Item #15.5 Construction phasing and Liquidated Damages: Phase 1 is
to be completed in 30 Calendar Days. Does the City have a pre-approved offsite source
for the required Crushed Structural fill? If yes, Please provide the test results and
location of the approved material. If not, Does the onsite rock material have to be
crushed and used as the Crushed Structural Fill, and select structural fill? This requires
the use of 191,400 CY of combined structural fill at the buildings, that means we need to
blast, size the material, crush, place and compact with testing, 6,380 CY per day. Will

Answer: There is no pre-approved offsite material source. It is the contractor’s
responsibility to meet the specifications at all times. Refer to Addendum No. 4 for
revised phasing. Liquidated Damages will be assessed at the completion of the

computed by the average end area method. Will you accept the DTM method of surface

Question No. 30: As stated in 152-2.2 first paragraph; until the work has been staked
out by the contractor, and the Engineer has obtained elevations and measurements of
the ground surface.
Does this mean that the Engineer will be performing this measurement and surveying for
obtaining these measurements and elevations?
Answer: Yes.
Question No. 31: If the plan holder list is available will you please have someone send
it?
Answer: The plan holder list can be found at www.sguconstruction.com.
Question No. 32: I have received Addendum #2 for the Finish Grading Project on CD,
but it is not in any format that I am able to open. Is there any other way I can obtain what
is on this disk?
Answer: Refer to Addendum No. 2. The files on CD released with Addendum No. 2 are
in AutoCAD format. The bidder will need AutoCAD or an AutoCAD reader to view the
files.
Question No. 33: Is there a bid schedule for the alternative work?

Answer: The bid schedule for the alternative work was released in Addendum No. 3.
Question No. 34: The cad file furnished as part of addendum 2 (file name:
1-x-AS-cont_fg.dwg) is missing some design topo in two areas.
1 - on the south end of the Vehicle Service Road (app 2,600 lf of road has no design
topo)
2 - SE of the building pad areas (contours 2823 thru 2828 are missing)
The area SE of the building pads can easily be drawn in, but the road section that is
missing would be a complete shot in the dark. Are you aware of this, and is there any
way this information can be distributed to bidders?
Answer: The finish grade contours are correctly shown. The “missing” portion is outside
of the Contract work area.
Question No. 35: During the job walk through of the site a question was asked: “Does
any of the material from the borrow site have to go to the south end of the project?”
Jacobs’ personal responded by saying no, all of the material in the borrow site will go to
the waste area. After analyzing the bid sheets, it seems that a substantial amount of
import is required to be placed in the South VSR. If the borrow site material is not
available, where is the import material to be obtained from?
Answer: It is anticipated that at least some of the material required to construct the VSR
will come from the GA Area. This is also the case for the south runway safety area
(RSA) if the alternate work is accepted.
Question No. 36: We were told that the PDF file would happen in Addendum #3. Is this
accurate?
Answer: Yes, refer to the CD supplied with Addendum No. 3.
Question No. 37: Is the rock blasting line item just for drilling and blasting (and not for
excavating, loading, hauling and placing the material)?
Answer: Yes, refer to the Bid Documents including but not limited to the Measurement
and Payment section of Specification P-152.
Question No. 38: Does the quantity for unclassified excavation include the crushed fill
material, select fill material rock blasting quantity? If yes, is the crushed fill material just
for supply and placement?
Answer: Refer to the Bid Documents including but not limited to the Measurement and
Payment section of Specification P-152.
Question No. 39: Item G-500 Erosion Control Measures sub-section 500-1.1 states:
"there are existing maintain as a part of the BMPs." Does the contractor have to
maintain all of the existing maintain as a part of the BMPs." Does the contractor have to
maintain all of the existing erosion control items that are in place or is it optional to
maintain these items?

Answer: The Contractor will be responsible for monitoring the existing BMP. After the
NTP is given the Contractor will have 14 days to inspect the existing BMPs and take
responsibility for weekly inspections. During the initial 14-day inspection, all necessary
improvements that will be needed per the existing approved SWPPP will be done by the
previous contractor. On the 15th, day the previous contractor will be issue a release of
responsibility.
Question No. 40: Special Conditions specification 9.10.3 states: “Immediately upon
completion, exposed surfaces shall be protected by re-vegetation or other appropriate
means." Does this specification refer to seeding or applying a tackifier to control the
levels of dust caused by the disturbance?
Answer: The seeding and tackifier will not be required with this project. However, the
contractor will be paid under specification item G-100.01 to control dust during the
project
Question No. 41: Item P-152-2.2.b - states to undercut Unsuitable Material and replace
with Suitable Material obtained from the grading operations. Will this replacement of
material be paid under the Unclassified Excavation item?
Answer: The required unsuitable material excavation will be paid in accordance with the
Bid Documents.
Question No. 42: Will rock blasting of borrow areas be paid under the rock blasting bid
item, if we run out of rock material in the excavation areas and have to borrow rock from
a borrow source or over-excavate the rock in the excavation areas to produce the
needed Crushed Fills?
Answer: We do not anticipate running short of rock. The required rock blasting will be
paid in accordance with the Bid Documents.
