Please complete the following application in its entirety. Only typed applications will be accepted. Use
only the space provided in the application to answer each of the questions; additional pages will not be
accepted. Each page in the workbook will be only one page when printed.
Use the "Tab" key to go to the next cell that will accept information. You may make hard returns within
the document by entering "ALT"+"ENTER" at the same time.
Do not copy and paste information from one cell to another.
Begin with page 1 and complete the pages in numerical order.
Make sure that all type is visible when the application is printed. DO NOT change the font size.

In addition, the following information is required to complete this application. To aid the State Office in
duplicating your application, please do not put your application in any type of folder, binder, etc. A
single staple or paperclip will suffice.
Please submit the following with your application:
        A. A completed, typed evaluation form from your FFA advisor recommending you for State
            FFA Office.
       B. A completed, typed evaluation form from a qualified reference which may include
            individuals who are NOT RELATED TO THE CANDIDATE, NOT AN FFA ADVISOR, AND
            HAVE KNOWN THE CANDIDATE FOR A MINIMUM OF THREE YEARS.
       C. A 250 word maximum typed statement by the candidate "Why I Desire to Serve as a
            State FFA Officer" on a separate sheet of paper.
       D. A copy of the applicant's final State FFA Degree application. This should include all
            required information. If a copy of the signed application is no longer available, a printed
            copy (without signatures) will be sufficient.
        E. A completed "State FFA Officer Qualifications and Prerequisites" with candidate's and
            parent/guardian's signatures. See pages 7 and 8 of this application.
        F. A photocopy of the insurance policy on the vehicle which the officer candidate plans to use
            during the State FFA Officer term of office.
       G. A completed "State FFA Officer Data Sheet" to be provided to all voting delegates. See
            page 9 of this application. Please make sure that the Data Sheet does not exceed one
            page in length.
       H. Schedule a "Pre-Interview" with the State FFA Office. The Pre-Interview dates are
            normally the first Friday and Saturday of May.
         I. Fill out the Medical Consent, Travel Agreement, and Dating Policy forms included in this
            application and make sure that they include all appropriate signatures.

The State Office reserves the right to videotape any and all of the interviews which take place during
the Nominating Committee process. If you have any questions concerning this application, please
contact the Arizona Association FFA at (602) 542-5564. Applications must be postmarked on or before
April 15th to be eligible to run for State FFA Office.

                    State FFA Officer Qualifications and Prerequisites

State FFA Officers are required to perform on a very vigorous and continuous basis. Therefore, it is
necessary that those who aspire to become officers are highly qualified, and able and willing to
perform. Please read and study the major qualifications and prerequisites very closely. When you are
fully convinced that if elected you will, without any reservations, be fully able to carry out the role and
responsibility of a State FFA Officer, sign and return this copy with your application.

                                 Role of the State FFA Officers

In order for present and future members of the State FFA Officer Team, Chapter Advisors State Staff,
and the membership-at-large to have an understanding of the role of State FFA Officers, the following
major areas of responsibilities have been defined as duties of the State FFA Officers.

1) The State FFA Officer shall be a member of the executive committee, which fulfills the specified
functions of governing the State Association.

2) The State FFA Officer shall execute the State Program of Activities and shall assist the Chapters in
the execution of their Program of Activities.

3) The State FFA Officer shall be a disseminator of specific Agricultural Education and FFA
information to the membership.

4) The State FFA Officer will motivate, inspire, and encourage FFA members to participate in
Agricultural Education and FFA programs to assist the members in their advancement of agricultural
knowledge and development of potential leadership abilities.

5) The State FFA Officer shall maintain positive relationships with members, agribusiness
organizations, educational organizations, personnel within the agribusiness sector, the public, and
others interested in Agricultural Education and the FFA.

6) The State FFA Officer shall project a positive image as a leader among American youth,
representing those qualities which members and society expect in a positive role model.

By submission of this application, state officer candidates understand that their information may be
used in publications throughout the period as a candidate as well as during the time covered as a state
officer and past state officer. The use of this information includes but not limited to information to be
used in conjunction with: Business cards/website/news releases (i.e. phone numbers, home address,

                                    Advisor's Evaluation Rubric

                                              General Information

 This rubric provides the scoring method for the Advisor’s Evaluation Form. The parts of the rubric that will be
graded include the 18 questions (Six traits with three questions each) as well as the Advisor’s Essay Response.

                                                      Questions
The questions that will be answered by the advisor(s) have all been formed in the same format as the questions
below. For each question, the advisor(s) will have the opportunity to rate the candidate according to the options
 of Always, Sometimes, or Never, choosing only one answer. In addition, the advisor(s) will also have the
             opportunity (optional) to justify their choices to the answers in 500 characters or less.

                 Johnny Greenhand has been eager at the chance to run for State FFA
                 Office since his junior year. Johnny has discovered what the job entails
                   by reading the job description on the National FFA website, and by
                 talking to several current and past State Officers. Johnny has read the
                guide to State Office on the Arizona FFA website and attended the State
                    Officer Workshop that followed the December COLT. Continually
                participating in his own chapter, Johnny proves that his desire to serve is

                                             Optional Justification
    If the Advisor(s) wishes to do so, they may include a justification for their answer selections in the space
     provided at the end. Any added information will help the Nominating Committee get a better idea of the
                                                    candidate.

                                            Essay Response Scoring
 For the Essay Response, a candidate can earn a maximum of 3 points and a minimum of 1 point. The essay
 allows for a maximum amount of 1500 characters. Essays will be awarded points based on how complete the
                             essay is and how well it portrays the candidate.



     All questions, the optional response to the questions, and the Advisor's essay
                response will be reviewed by the Nominating Committee.

                                       Introduction to the Evaluation
 This form contains a series of questions for the chapter advisor(s) to answer regarding a State FFA Officer
candidate. The questions have been formulated in accordance to the six main traits that will be evaluated by
  the members of the Nominating Committee at the annual State Leadership Conference. The Nominating
                          Committee will see this form and review it appropriately.
                                         Answering the Questions

 For each question the advisor will select one answer of either Always, Sometimes, or Never with the most
points given for the answer of Always and the least points for an answer of Never. Following each question, a
   comment section is available to justify the reasoning of each answer with a maximum of 500 characters
  allowed. One evaluation form may be submitted per candidate. Advisors of the same chapter in which the
candidate is from, who have contact with the candidate, may complete this form together, but regardless, only
                                one form may be submitted for the candidate.

                                          Advisor's Essay Response
       Following the questions there is an Essay Response section for the advisor(s) to complete. This
           section allows the advisor(s) to write freely about the candidate, providing the Nominating
 Committee with any additional information that the advisor(s) feel the Nominating Committee should have.
 This is the advisor’s opportunity to provide the committee with a better understanding of the candidate. This
                                 section allows a maximum of 1500 characters.
                                                      Grading
          This form will be graded according to the Advisor’s Evaluation Rubric which can be found at
                            www.azffa.org under downloads, State Officer Candidates.

                                              General Information
 This rubric provides the scoring method for the Reference Evaluation Form. The parts of the rubric that will be
    graded include the 18 questions (Six traits with three questions each) as well as the Reference's Essay
                                                   Response.

                                                      Questions
 The questions that will be answered by the candidate’s reference have all been formed in the same format as
 the quest below. For each question, the reference will have the opportunity to rate the candidate according to
the options of Always, Sometimes, or Never, choosing only one answer. In addition, the Reference will also
         have the opportunity (optional) to justify their choices to the answers in 500 characters or less.

                                           How Points are Awarded
Each answer will be scored as follows:
Always: 3 Points
Sometimes: 2 Points
Never: 1 Point

                                            Optional Justification
    If the Reference wishes to do so, they may include a justification for their answer selections in the space
     provided at the end. Any added information will help the Nominating Committee get a better idea of the
                                                   candidate.

                                       Essay Response Scoring
 For the Essay Response, a candidate can earn a maximum of 3 points and a minimum of 1 point. The essay
 allows for a maximum amount of 1500 characters. Essays will be awarded points based on how complete the
                             essay is and how well it portrays the candidate.



     All questions, the optional response to the questions, and the Advisor's essay
                response will be reviewed by the Nominating Committee.

                                       Introduction to the Evaluation
 This form contains a series of questions for the chapter advisor(s) to answer regarding a State FFA Officer
candidate. The questions have been formulated in accordance to the six main traits that will be evaluated by
  the members of the Nominating Committee at the annual State Leadership Conference. The Nominating
                          Committee will see this form and review it appropriately.
                                         Answering the Questions

For each question the reference will select one answer of either Always, Sometimes, or Never with the most
points given for the answer of Always and the least points for an answer of Never. Following each question, a
   comment section is available to justify the reasoning of each answer with a maximum of 500 characters
                        allowed. One evaluation form may be submitted per candidate.

                                       Reference's Essay Response
    Following the questions the Reference may write freely about the candidate, providing the Nominating
 Committee with any additional information that the reference feels the Nominating Committee should have.
This is the reference's opportunity to provide the committee with a better understanding of the candidate. This
                                section allows a maximum of 1500 characters.
                                                  Grading
       This form will be graded according to the Reference's Evaluation Rubric which can be found at
                          www.azffa.org under downloads, State Officer Candidates.
