Congratulations on your new baby! By bringing your newborn to see the doctor you are getting your child off to a healthy start. It is important that you continue to bring your baby back for regular well-child exams and immunizations. If you have not yet enrolled your new baby with your insurance, follow these instructions: If you have Public Aid: 1. Contact your caseworker at your local DHS office within 30 days of your baby’s birth 2. Provide the caseworker with the following information the day your baby was born first and last name of your baby where your baby was born the case number from your medical card (it is an 11 or 13 digit number, usually beginning with a 94 or 96) If you need the phone number to your local office, please see our Insurance Counselor, Benny (phone number 650-3243). He will provide you with the number or any other reference for contacting your local office. If you have an HMO or insurance other than Public Aid: Notify your insurance carrier or employer to add your baby as soon as possible, with an effective date of your baby’s birth date. If you have an HMO, be sure to select a Lawndale Christian Health Center physician as your baby’s primary care physician. Simply having a policy does not ensure that your children are covered by that policy. It is your responsibility to add your new baby to your insurance policy.
