NATIONAL PARK SERVICE GLEN CANYON NATIONAL RECREATION AREA APPLICATION FOR COMMERCIAL PHOTOGRAPHY/FILMING PERMIT In response to your initial inquiry concerning commercial photography/filming at Glen Canyon National Recreation Area, we are enclosing a worksheet for you to complete and return to us. It is important that the worksheet be as detailed as possible concerning your proposed activity. For example, we must know where you propose to shoot, what type of equipment you will use and any special effects you may wish to use. You will not be permitted to arrive and then select locations on the day of shooting. If you are not familiar with the area, it is advisable to make an advanced scouting trip for locations. If you would like a ranger to accompany your scout, you will be responsible for the costs of that service. The National Park Service staff will use the information on the worksheet to evaluate the impact of your activity on park resources and other visitors. It may require several weeks to review your request and render a decision. All applicants must submit a $100 non-refundable application fee with the worksheet, whether or not a permit is issued. If your request is approved, depending on the scope and size of the proposed activity, a $200 administrative charge may be required to cover expenses and a monitor (at $80/hour with a 3-hour minimum per day) may be required to oversee the activity. Additionally, daily location fees will apply (see schedule below). Payment of fees shall be completed by credit card (Visa, Mastercard, Discover, American Express, and Diner's Club). If you are unable to pay by credit card, please provide documentation and a policy waiver may be granted. If your request is approved, a certificate of insurance will be required showing you have general liability coverage in a minimum amount of $1,000,000 (U.S. currency). The certificate must be written by a U.S. company and must name the United States Government/Glen Canyon NRA (see address below) as an additional insured. We will need an original copy of this certificate for our files. A performance deposit may also be required for your activity; the amount of which will be determined from the information provided on the worksheet. This deposit will be returned to you, upon completion of filming, if all accrued costs have been paid, stipulations have been met and the resource has not been damaged. The use of aircraft in connection with this permit may require a Certificate of Waiver, issued by the Federal Aviation Administration, granting a waiver of FAR 91.119, Minimum Safe Altitude. If a waiver is required, a copy of the waiver must be provided to us. General liability insurance coverage totaling $3,000,000 and a minimum performance deposit of $25,000 will be required for all photography/filming utilizing aircraft. Please submit the completed worksheet along with a credit card number for the $100 application fee to the Permit Coordinator at the address below. If you prefer, you may call with a credit card number but please note that your application will not be processed until this is received. Glen Canyon National Recreation Area 691 Scenic View Drive P.
