Order #_______________Date:___________ , Authorize the estimate or repair work set forth herein, to be completed by CAE
SOUND (Hereina er referred to as "CAE"). e Customer agrees and acknowledges that, CAE, is not responsible for loss or damage to
e Customer's equipment or accessories accompanying the equipment including, but, not limited to any acts of God that are beyond
the control of CAE, as well as any delays caused by unavailability of parts and / or materials. e Customer also authorizes CAE, its
agents and its representatives to operate the equipment herein described for the purpose of inspection, testing or QC burn in.
CAE shall have an express lien on this equipment as security for all charges incurred, until paid in full, including any other
e Customer is responsible for all charges incurred including, but, not limited to the following: estimate or diagnostic charges,
repair charges, shipping charges, phone, FAX, and COD charges, etc.
Estimates must be requested at the time the Work Order is written. Our Minimum charge will be collected to cover the expenses
of diagnosing the malfunction.
Designated jobs are covered by our 60 Day Limited Warranty. See CAE Parts and Labor Warranty Policy.
Please be aware that the contents of memory may at times be lost when the unit is repaired or when a malfunction has occurred.
Important data should be stored on an external medium such as RAM CARD or DISK, or written down on paper. During repairs, due
care is taken to avoid the loss of data, however, in certain cases we regret that it may be impossible to restore the data.
We are not responsible for lost data. PLEASE BACK UP YOUR DATA CAREFULLY!!
Inspection estimates cover disassembly, testing, diagnosis, and reassembly of the equipment.
CAUTION: THE ACT OF DISASSEMBLY MIGHT PREVENT THE RESTORATION OF THE UNIT TO ITS
FORMER OPERATING CONDITION.
Standard or advertised labor or parts prices do not apply when the condition of the equipment is found to be such that additional labor
or parts are required to complete the agreed upon work; or for us to guarantee our work; or in our opinion, to leave the equipment in
safe operating condition; or equipment is not stock or has been modi ed
CAE does not guarantee parts, materials or so ware supplied by the Customer and o ers No Warranties of such materials.
STORAGE FEES of $10.00 / day WILL BE CHARGED ON ALL UNITS LEFT OVER 30 DAYS. EQUIPMENT LEFT
OVER 90 DAYS WILL BE SOLD. A Service charge of 1.5% per month, 18%APR, will be added to all overdue accounts. e
Customer will also be liable for all legal and collection costs.
