Accident Incident Reporting Procedures for UBC Library

GENERAL INFORMATION:

All incidents/accidents must be reported to a supervisor as soon as possible following the
incident/accident. The main reporting forms used are:

       UBC Faculty & Staff Incident/Accident Report
       WorkSafeBC Form 6A- Worker’s Report of Injury or Occupational Disease to the Employer
       WorkSafeBC Form 7A- First Aid Report (completed only when first aid is administered by a
        qualified FA attendant)
       UBC Student & Visitor Incident/Accident Report

As of January 1 2008 workers who are injured at work and miss time from work as a result can now
report their injuries to WorkSafe BBC calling the Teleclaim Contact Centre by dialing 1-888-WORKERS (1-
888-967-5377), Monday to Friday, 8 am to 4 pm. Through Teleclaim, callers speak to a WorkSafeBC
representative, who completes an injury report on the workers behalf, explains the claim process, and
helps obtain the services to assist with recovery and return to work.

If you have sustained a workplace injury, or feel that your work environment causes your present
symptoms the following steps must be taken:

STAFF ACCIDENT/INCIDENT/NEAR MISS REPORTING INSTRUCTIONS

EMPLOYEES:

     1. Seek appropriate first aid or medical treatment (if first aid is administered, a WorkSafeBC Form
        7A- First Aid Report should be completed by the first aid attendant)
     2. Notify your supervisor or department administrator immediately to ensure the completion of a
        UBC Faculty & Staff Incident/Accident Report.
     3. In all cases, UBC requires injured/ill workers must complete a WorkSafeBC Form 6A- Worker’s
        Report of Injury or Occupational Disease to Employer

If you require medical treatment, see a physician and inform him that your injury/illness may be work
related.

REPORTING INSTRUCTIONS FOR SUPERVISORS:
   1. The supervisor must complete all applicable Sections (Sections 1,2,3) of Page 1 of the UBC
      Faculty & Staff Incident/Accident Report form. Page 2 - accident investigation (Section 4) must
      be completed by the area supervisor and a worker safety representative within 72 hours after
      the incident has been reported.
   2. Supervisor will request the employee to complete a WorkSafeBC Form 6A- Worker’s Report of
      Injury or Occupational Disease to Employer
   3. Supervisor will send the completed originals (along with any supporting documentation) via
      internal mail to:

                    IKBLC Library Units: Library HR
                    All other IKBLC-Based Departments: HSE

    4. Supervisor will fax copies to Sandra Singh, Director, IKBLC at (604) 822-3242.
 For Library Units: Library Human Resources
      1. Compiles the information for Health, Safety & Environment and maintains a record in the employee file
      2. Notifies local safety committee Chair
      3. Maintains record(s) for Library Administration
      4. Acts as liaison between the employee’s supervisor, the injured employee and WCB/HSE
      5. Provides follow-ups with local safety committee and ensures that the appropriate information is posted
           on staff bulletin board


  For other units, your Faculty/Department’s OHS Coordinator will provide IKBLC based equivalent services.

Campus First Aid: For first aid treatment phone 604-822-4444 (or 2-4444 from a University telephone).
First Aid at UBC is a free service for all employees that is available 24-hours a day. The service is
contracted through the Vancouver Fire & Rescue Service.

UBC STUDENT/VISITOR ACCIDENT/INCIDENT/NEAR MISS REPORTING

Students and visitors include all non-UBC employees present or working on University premises. These
include undergraduate students, graduate students (except when employed as teaching or research
assistants), visitors and members of the general public.

If a student/visitor is injured or becomes ill at one of the UBC Library sites, a UBC Student & Visitor
Incident/Accident Report must be completed. If the student/visitor is unable to complete the form, the
supervisor on duty or witness would complete the form on their behalf.

    1. UBC Library Units will send original to Deborah Austin, Director, Human Resources, Koerner
       Library – Floor 2
    2. All other IKBLC-Based Units will send original to their Faculty OHS Coordinator.
    3. Send copy to the Director, IKBLC (site of injury), who will see that a copy gets to the Building
       Safety Committee.
    3. Send copy to Health, Safety and Environment (50 – 2075 Wesbrook Mall, Campus Mail Zone 1,
       Fax: 604-822-0572)
    4. Send copy to Risk and Insurance Manager (2336 West Mall, Campus Mail Zone 4 Fax 604-822-
       1224)
    5. Fax a copy to IKBLC’s Land and Building Services Facility Manager (Fax: 604 822-2334)
