                                             CONTRACT/CONFLICT ATTORNEY
                                                       SURVEY
The purpose of this survey is to determine the qualifications of attorneys accepting cases assigned by OPD,
their access to support staff, and their caseload. This data will enable OPD to evaluate contract attorneys and
assist in developing a reasonable rate structure for payment for attorney services and reimbursement of
associated costs.

The Statute governing the awarding or assigning of cases to attorneys is set forth in Section 47-1-216, MCA.
Failure to complete and return this survey may delay future assignment of cases. Please return no later
than April 16, 2007. Attach additional sheets as needed. If you prefer to submit electronically, the survey is
