Supplemental Medical Plan?

              A Supplemental Medical Plan


Is a fully insured plan that allows an employer to provide benefits
to key employees for most medical, vision, hearing and dental
expenses not otherwise covered by their existing benefit plan.

             IRS Guidelines


The IRS guidelines define an insured plan as one that is
underwritten by a licensed insurance company with the
insurance company assuming a portion of the risk. The
transfer of risk is accomplished through a stop loss provision
that caps the employer company’s liability at a percentage of
the maximum medical reimbursement benefits for all
participating key employees.

           Tax Advantages

There are several tax advantages to a properly structured
insured plan.
  Insurance premiums can be tax-deductible to the employer
   and non-taxable to employees
  Insurance benefits are generally received income tax free.


 It is strongly recommended that interested parties must seek
 the advice of tax counsel when considering adopting an
 Insured Medical Supplemental Plan.

The employer selects the desired classification for each
participating key employee and his/her eligible dependents
from the plan schedule:

                Simple to Install


Initial Enrollment
The employer completes the application and adoption agreement.
With the application submit the annual participating premium,
enrollment forms and underlying health plan documents.
The above information will be reviewed, processed and installed if
accepted.

               Claim Submission


After the key employees’ claims have been processed per the term
of the underlying plan, the key employees’ unpaid expenses are
submitted to the employer. The employer submits the claims and
the additional premiums to BeniComp, Inc. We will send a benefit
check to key employees for the covered expenses. It is that
simple.

              Underwriting Guidelines

Eligibility
      A minimum of two covered participants for BCS and one for
       Executive Edge
      Actively at work
      Covered by an underlying plan
      $5,000 for BCS and $5,000 for Executive Edge deductibles
       are the maximum underlying plan deductibles
      $1,000,000 minimum lifetime maximum
      No unusual gaps in underlying plan
      Eligible dependents must not be hospitalized on effective
       date
      A balanced class structure is required

               Underwriting Guidelines

Eligibility
   Coinsurance (non-PPO plan): not to exceed 20% of the first
   $10,000 of eligible charges beyond the deductible each year
   Co-insurance (PPO plan): not to exceed 20% for preferred
   network services or 40% for non-preferred network services of
   the first $10,000 of eligible charges beyond the deductible
   each year
   There is a per occurrence limit of 10% of the annual maximum
   for infertility on BCS and a 10% per diagnosis per year on
   Executive Edge
    Vision and Dental are not required

            Stop-loss features

When the total premiums paid in a plan year exceeds the
maximum premium, Assurity Life Insurance Company will
refund the excess.

                        What’s Covered ?
       Some Covered Benefits
       Generally, if an expense is medically necessary and qualifies under
       Section 213 of the Internal Revenue Code, it would be eligible for
       reimbursement under this insurance plan. Some covered charges
       include:
           Prescription drugs
             Private-duty nursing
             Hospital expenses, including private-room charges
             Dental and orthodontic expenses
             Home health care
             Alcoholism and drug-abuse treatment and facilities
Note: Individuals must be under the care of a legally qualified physician to receive reimbursements.

             Vision Care including all types of frames
             Inpatient and outpatient psychiatric care
             Charges for the diagnosis of infertility
             Charges for the treatment of infertility up to 10% of annual
              maximum per calendar year
             Medical transportation, including air services and hotel
             Medical supplies and equipment


Note: Individuals must be under the care of a legally qualified physician to receive
   reimbursements.

 Hospitalization, services, treatments or supplies furnished by the
  U.S. or foreign government agency, unless otherwise prohibited
  by law
 Service contracts or warranties relating to vision care
 Custodial Care
 Accident or illness for which the individual is entitled to benefits
  under any worker’s compensation or occupational disease law

                    What’s not covered?

    Health Club dues or exercise equipment
    Blood Storage
   Hospital charges for confinement in a
     long-term care unit or skilled nursing facility
     unless confinement commences within 14
     days after discharge from a qualifying
     hospital confinement



No benefits are payable unless the individual is under the direct care of a legally
qualified physician for reasonable and necessary treatment.
