constitutes a violation of the Regulation and the License.

       Action: The Respondent must submit corrective action measures to the
Department and to pay a civil penalty of three thousand dollars ($3,000.00). The
Department suspended the Respondent’s Permit for a period of thirty (30) days, as
required by the Regulation. The corrective action measures have been submitted
to the Department and approved, the penalty has been paid and the Permit has
been reinstated.

held, as often as necessary so as to control insects and other vectors; (2) did not
remove all putrescible wastes for proper disposal within twenty-four (24) hours of
receipt; (3) did not clean the tipping floor at the end of the working day; and (4)
did not have an operator present at the facility during all operating hours. This
