   b. Obtain the MDS 3.0 – Place in electronic
   folder

     revised Nursing Home Assessment Tool –
     Rand Report (separate out from MDS)
     iii. MDS crosswalk
   c. Inform staff in (1a) re: location of “Rand”
   reports and MDS 3.0
   d. Designate staff/consultants to serve as
   experts for each section of the MDS. List and
   person expert (Note: This is a good way to
   evaluate the assessments/manual or
   electronic.)

    process , including the MDS change, i.e.

      does what, by when, how, down to date entry

      resident/family/responsible party
5. Assessment Evaluation

   a. Review the manual of electronic assessment
   tool – Crosswalk items and intent against the
   MDS 3.0 (use the crosswalk)

   b. Identify items on the MDS 3.0 vs. local
   clinical assessments (manual and automated)
   where additional comments aside from “����         ”,
   “yes/no” items would clarify resident status
      i. Nursing (i.e., pain, fall/risk, cognition (Brief
      Interview for Mental Status[BIMS]),
      mood/behavioral items/psychotherapeutic
      drugs, nutrition, ADL, memory (especially
      explaining fluctuating capacity), etc.
      ii. Nutritional
      iii. Social
      iv. Activities
      v. Therapy
      vi. Physician H&P / Progress Notes
      vii. Identify – other assessments (Confusion
      Assessment Method [CAM], Geriatric
      Depression Scale [GDS])

content.

review of templates, administrative etc. (see notes
later).
