Dear New Gustie:

A loose translation of this German saying suggests that we're besieged by forms from the cradle to the grave.
Somewhere in the middle of that continuum is Gustavus Adolphus College, the academic community and campus
family into which we look forward to welcoming you very soon.

Well-known for its friendliness, Gustavus tries to keep form filling to a minimum. Particularly as new students
matriculate at the College, however, even we find the use of various forms helpful and even necessary in meeting
students' needs as well as possible.

Enclosed in this packet are both informational materials and assorted forms that must be returned to the College
by July 1. Please consult the "Gustavus Enrollment Checklist" to be sure that you have all the listed items; if any
are missing, call Sara Orth or Amy Braun at (507) 933-7502/6244 and the items will be sent to you promptly.

Ready? Please read each of the descriptive paragraphs below, carefully follow the instructions where appropriate,
and return all requested items by July 1. Forms may be mailed to Gustavus in the pre-addressed envelope
provided or turned in at June Registration. A Student Accounts representative will be available during on-campus
registration in June to collect forms and to notarize the Statement of Financial Responsibility.

Housing
        The "Housing Contract and Handbook” and with it, the “Preference Questionnaire” should have been sent
to you under separate cover along with instructions as to when to return the latter. If you have not received these
materials, please phone (507) 933-7676 to request them.

Student Property Insurance
         The College's property insurance will not cover the personal effects of students (audio and computer
equipment, clothes, and other personal items) in the event of fire, theft, etc. Students should therefore consult
their parents or guardians regarding the availability of coverage under their homeowner's or tenant's policy. If
parents/guardians have no insurance coverage, but coverage is desired, students should purchase personal effects
coverage, at their expense. For information on student personal effects property insurance you may wish to
consult National Student Services, Inc. at (800) 256-6774 or www.nssinc.com.

New Student Information
         The "2009 New Student Information Form" (lavender) requests a variety of information to assist the
College's Office of Institutional Advancement. Information you provide here may be used in the preparation and
distribution of College statistics, the identification and preparation of grant proposals, and College relations with
parents and families.
Student Conduct Code
        Enclosed for your review are expectations that have been established by Gustavus over the years for the
members of its student community. By adhering to these basic expectations Gustavus students acknowledge their
membership in a community of learning where individual lives are inextricably tied to and influenced by others
around them. Please review the "Student Conduct Code" and "Expectations of Student Conduct", discuss them
with your parents/guardians, and if you have questions, please feel free to call us. After you have reviewed the
above, please sign, date, and return the white copy. You may keep the yellow copy for your records.

Financial Responsibility
         The "Statement of Financial Responsibility” (SFR) is required of each student before the first day of class
attendance. This form indicates who, in addition to the student, is responsible for the payment of fees and makes
it possible for the College to offer a variety of payment options to students and parents. Both the student and at
least one parent/legal guardian or guarantor must sign the SFR in front of a Notary Public unless the student is
married or at least 24 years of age. Please sign, date, and return the white copy of the SFR. You may keep the
yellow copy for your records. For more information about the SFR, consult the "2009-10 Payment Schedule" that
you should have received already. A notary will be available if you wish to complete this form at registration in
June. To take advantage of this service please be sure to bring the form and a picture ID with you and do not sign
the form until you come to June registration if you intend to complete it at that time.

Disability Accommodation
         Please be assured that no otherwise qualified person with a disability (attention deficit disorder, learning
disability, physical impairment, etc.), on the basis of that disability, will be excluded from participation in, be
denied the benefits of, or otherwise be subjected to discrimination under any program or activity at Gustavus
Adolphus College.
         As part of our effort to facilitate accessibility of College-sponsored programs, we actively solicit direct
communication from individuals with documented disabilities as early as possible. This will facilitate early
planning which is critical to our accommodation efforts. If you wish to be contacted by a resource person for

         Examples of available assistance include physical accessibility modifications, assistance with obtaining
readers/writers/interpreters, and help in arranging for appropriate testing.

Meal Plan Contract
        A Meal Plan Contract is required of all students residing in campus facilities other than College-owned
houses and apartments. Please review the three options carefully, select a First-Year Student Declining Balance
Meal Plan Contract, sign and date the form and return it along with other forms.

Return all items requested above either in person at June Registration or via mail by July 1 in the envelope
pre-addressed to Gustavus.

Future Mailings Coming to You from Gustavus:
       June:    Enrollment Deposit Billing – mailed on or about June 4;

        July:     Refund Policy and information about the Optional Dewar Insurance for medical withdrawals;
                       Fall Semester Fee Statement--mailed on or about July 30th;

        August: Housing assignments; mailed during late July/early August;
