PART 1: START: Chair submits request to Dean to start recruitment
      Step 1       Department Chair completes Faculty Search Request Form (APPENDIX A). Forwards to Dean.


      Step 2       Dean approves request, forwards original to Director, F&A for review and assignment of search tracking
                   number.
      2-A          Director, F&A forwards original form with search tracking number to the Dean's Coordinator.
                   Dean's Coordinator prepares Memo of Search Approval (APPENDIX D), sends to Department Chair.
                   The Memo indicates Dean's approval of the Faculty Search Request and includes the assigned
      2-B          search tracking number. Recruitment efforts may begin. Copies via email:
                        Department Administrator, A-Dean, F&A, Director, F&A

                   Director, F&A assembles SOP Faculty Search Packet (APPENDIX E) and forwards it via email to the
      Step 3
                   Chair of Search Committee. Copies via email:
                     Chair, Dept Administrator, A-Dean, F&A, Dean's Coordinator



Part 2. SEARCH CONDUCTED BY SEARCH COMMITTEE


Part 3. SEARCH COMPLETED: Recommendations and Preliminary Offer
                   Committee selects finalist,. Committee Chair assembles and forwards the RECOMMENDED New
      Step 4
                   Faculty Appointment Packet to Department Chair (APPENDIX B).

                   Department Chair approves recommendation, drafts preliminary offer proposal to include starting salary
      Step 5       and draft Faculty Start-Up Package Form (APPENDIX C). Discusses proposal with Dean; gets
                   approval to begin negotiations with selected candidate.

                   Department Chair negotiates with candidate; sends written Preliminary offer letter. Chair requests
      Step 6       candidate's CV which must be signed, dated and confirmed to be true and accurate. Copies:
                     Dean, Dept Administrator, A-Dean, F&A, Director, F&A

      Step 7       Candidate accepts Preliminary offer; endorses letter; returns it to Department Chair.



PART 4: APPOINTMENT PROCESS
                   Department Chair assembles and routes PRELIMINARY New Faculty Appointment Packet for
      Step 8
                   President (original and one copy) (see APPENDIX B) to Dean's Coordinator (Mimi Wasti)

PART 4: APPOINTMENT PROCESS (Continued)
                      Dean's Coordinator processes PRELIMINARY Packet for distribution:
       Step 9          A. Within 24 hours of receipt: Original to: Director, F&A Copy: Dean
                       B. Copies of Faculty Start-Up Package Form to: Chair, Depart Administrator, A-Dean, F&A

               Director, F&A reviews PRELIMINARY Packet for completeness and accuracy. Ensures:
                 A. All SOP/Internal documents are included
                 B. All Campus required documents are included
                 C. Within three working days proceeds with one of two courses of action:
                     (1) Packet complete, adds FAIS form, forwards to Dean's Coordinator for processing. See Step
       Step 10            12.
                     (2) Packet not complete, holds Packet, documents deficiencies and communicates to:
                              Department Chair, Department Administrator, Dean's Coordinator
                          Sends weekly updates of unresolved deficiencies to:
                              Department Chair, Department Administrator, Dean's Coordinator

               Dean's Coordinator receives PRELIMINARY Packet from Director, F&A for processing. Ensures:
                A. Faculty Start-Up Package Form is completed and approved, removes from Packet
                B. Assembles the FINAL New Faculty Appointment Packet for President (A PPENDIX B) with
                    documentation in correct order.
       Step 11  C. Adds Dean's Recommendation letter (original) on top of Packet. Packet is now FINAL.
                    Submits number of copies of FINAL for who????
                D. Makes XFINAL New Faculty Appointment Package to the VPAA for approval.
                E. Dean's Coordinator communicates delivery of Packet via email to:
                    Department Chair, Department Administrator, Director, F&A

       Step 12 Dean's Coordinator monitors the status of submission to VPAA . Two weeks after submission - checks
               status of submission. Reports status and issues to:
                     Department Chair, Department Administrator, Director, F&A

       Step 13 President approves appointment, communicates to Dean

               Dean's Coordinator prepares appointment letter for Dean's signature then sends letter and FAIS to
       Step 14 candidate. Dean's Coordinator distributes:
                 Copies: Department Chair, Department Administrator, A-Dean, F&A, Director, F&A, SOP PR-HR
                 Packet: (originals and copies) of essential search documents for candidate's employee file to SOP PR-
                 HR.

Part 5: CLOSE SEARCH
       Step 15 Department routes recruitment documentation to Director, F&A for file retention by SOP PR-HR.

Part 6: PAYROLL PROCESSING
       Step 16
                      Department Administrator has responsibility for submitting complete and accurate payroll instructions to
                      the School Payroll-HR Office. CRITICAL: The Employment Eligibility Verification Form (I-9 Form) must
                      be completed by employee within three days business days of the date employment begins

Department Chair sends the following to the Dean's Coordinator:

At this time the Department Chair must also forward the following documents to the Dean. These items are for internal
purposes only and are required for a complete Preliminary request package to be complete

FINAL: New Faculty Appointment Package from Dean to President
Dean submits Faculty Appointment Package to Office of Academic Affairs for approval
Package must be complete, all documents signed, and organized as follows:
