Instructions:

 1. Please completely fill out the demographic information or we will not be able to include your information in the survey compilation.
 2. Please include a wage range ("minimum", "maximum" and "average") for each category of employee where there is more than one individual. If there is more than one
 individual in the category, but the "minimum", "maximum" and "average" are all the same, include the same number in each box. If there is a position held by only one
 person in your company, please enter their actual wage or salary amount in the "average" box only for that category. If there is no one in a category, leave all boxes blank.
 Even if your company has a standard pay grade range, please report the actual wage range as opposed to the standard.
 3. To streamline the survey, individual job categories have already been placed into an hourly or salaried section. If your compensation is structured differently, use 2,080
 hours to convert an hourly position to salary or vice versa (e.g., if your "Maintenance Supervisor" is paid $40,000 salary, please divide $40,000 by 2,080 to report an
 average wage of $19.20).
 4. Please present part-time employees as full-time, annualizing out the salary or wages based on 2,080 hours/year.
 5. You do not have to fill out the entire survey form if you do not have employees in the categories indicated. However, please match your workforce as closely as possible
 to the categories in this survey, recognizing titles of similar responsibilities.
 6. Average salaries should be given as the mean (average of all values), not the median (middle value).
 7. Direct wages will be compiled revealing the mean, lowest and highest wage range for each category.
 8. The benefits section will be shown as a percentage of respondents that offer each benefit for each category of employee.
 9. Incentive compensation will be compiled revealing the mean, lowest and highest incentive compensation range for each employee category. Incentive compensation is
 that above and beyond the employee's wage rate or salary as you have indicated it on the survey, listed as a % of the base salary.
 10. Please fill out a separate form for each region and/or division.
 11. Canadian firms please convert responses to U.S. dollars using a conversion rate of $1 Canadian dollar = $0.94 U.S dollars.
