                                            EFS Standard Codes - General Information
The Agency for Workforce Innovation (Agency) distributes a list of standard codes for specific data fields in the state‟s Enhanced Field System (EFS). In
accordance with section 11, paragraph a)1) of the Grants, Terms, and Conditions the coalition must comply with the definitions contained in the most current
version of the Agency‟s Office of Early Learning EFS Standard Codes document. The Agency has established the standard codes to ensure uniform data
reporting. Standardization of codes enables us to report accurately on local, state and federal reports. The Agency updates, maintains and monitors the master
list of codes and distributes updated lists to each coalition when changes take place.

NOTE: Each early learning coalition is required to enter in EFS the standard codes in this document. Unless otherwise directed, standard codes should be
updated within two weeks of the release of a new standard code document.

The EFS Standard Code document includes code tables with standard codes only and code tables that may include custom codes. A coalition may not change or
add custom codes to the standard code tables – only the Agency may change these codes. The Standard Code document also includes code tables that can
include custom codes; these tables are highlighted in BLUE. Additional codes may be added to any table in BLUE with coalition approval. To activate a code, type
a „Y‟ in either the CCMS or CCRR column.

Do not delete an old code or an old code description; enter "DO NOT USE" in front of the code description and inactivate the code by typing „N‟ in the CCMS or
CCRR column.

A      AFFIDAVIT                                       If no other documents are available, you may accept an affidavit of age sworn to by the parent and
                                                        accompanied by a certificate of age signed by a public health officer or by a licensed practicing
                                                        physician which states that the physician has examined the child and believes that the age as stated
                                                        in the affidavit is substantially correct. Enter a history note explaining why an alternative proof of age
                                                        was not available and what efforts were made to obtain such documents
B      BIRTH CERTIFICATE                               A duly attested copy of the birth record for the child and filed according to law with a public officer
                                                        charged with the duty of recording births. May include a copy of the record produced or issued by the
                                                        hospital where the child was born.
C      COURT RECORD                                    A court order or judgment specifying the child‟s age or date of birth
D      DRIVER LICENSE                                  Any state-issued or foreign government issued driver license. Must include the client's picture.
H      HEALTH RECORD                                   An immunization record indicating the date of birth, signed by a public health officer or by a licensed
                                                        practicing physician. This includes Florida SHOTS record.
I      INSURANCE RECORD                                An insurance policy on the child‟s life that has been in force for at least 2 (two) years
J      GOVERNMENT ID                                   A copy of a federal or state issued picture identification including state ID, alien registration card,
                                                        refugee care, employment ID, student ID. May include documents issued by other governmental
                                                        agencies or foreign countries.
M      MILITARY                                  
       IDENTIFICATION                                   A valid military dependent identification card
N      NOT VALIDATED                                  May be used for wait list (if not required) clients, TAPP, or protective services clients who do not
                                                        present with a record of birth at intake. Protective service clients should be updated when
                                                        documentation is made available.
NP     NON PHOTO ID                                    A client may present a non-picture ID, but must then present a second form of identification.
P      PASSPORT                                       A passport or certificate of arrival in the United States showing the age of the child

R      RELIGIOUS RECORD                              A bona fide contemporary religious record of the child‟s birth accompanied by an affidavit sworn to by
                                                      the parent or a copy of a certificate of baptism showing the date of birth and place of baptism.
S      SOCIAL SECURITY CARD                          Requires a second form of identification showing the client's name such as pay stub, employment
                                                      record, insurance card, or other documentation
T      TRANSCRIPT                                    A copy of the recorded age shown in the child‟s school record at least four (4) years prior to
                                                      application, stating date of birth
Note: You may not deny services to at-risk children while waiting for the necessary documentation to verify the child‟s age. However, this requirement
does not relieve the coalition or its contractor from obtaining the documentation.
Note: to increase the character length for the new NP code, go to System Admin>Reference Table Maintenance>Reference Table Definition and change
Length of Code to 2 characters.

                                     A lifelong developmental disability, which affects communication and behavior and which usually, appears before age
                                     three. It is characterized by lack of meaningful speech or inappropriate speech; withdrawn, anti-social and/or affectionless
                                     behavior; a fascination with objects rather than people; prolonged odd body movements; a hypersensitivity to stimuli;
             AUTISM SPECTRUM         stereotypic and compulsive behavior; and a failure to initiate or relate to people.(i.e. Asperger Syndrome, Autism,

                                     A disorder in which developmentally inappropriate inattention and impassivity are exhibited. Some characteristics are: not
                                     staying on task, difficulty organizing and completing work, inability to stay with activities for periods appropriate for child‟s
                                     age, failure to follow through on parental requests. Symptoms may vary with situation and time, i.e. home, school, groups,

                                A condition in which the individual has an acquired hypersensitivity to substances that normally do not cause a reaction.
                                Manifestations most commonly involve the respiratory tract or skin and include eczema, hives, nasal discharge and

                                     An inherited disease that affects the pancreas, respiratory system and sweat glands, which usually begins in infancy and

                                      A disorder in which the pancreas produces too little insulin with the result that the body is unable to adequately metabolize
                                      sugar. Principal symptoms are elevated blood sugar, sugar in the urine, excessive urine production and increased food
                                      intake. Complications of diabetes if left untreated include low resistance to infections leading to a susceptibility to
                                      gangrene, cardiovascular and kidney disorders, disturbances in the electrolyte balance and eye disorders, some of which

                                      A defect in one or more parts of the ear and its associated nerve pathways that lead from the ear to the brain which

             SPEECH/LANGUAGE          disabilities fall into several categories; articulation problems in which speech sounds are omitted, replaced by substitute

                                      Uncontrolled movements of the muscles of the body or change in alertness or behavior characterize seizures. They are
                                      caused by certain abnormalities in the brain. In the normal brain, there is organized electrical activity, which is always
                                      present. A seizure happens when bursts of unorganized electrical impulses interfere with the normal brain electrical
                                      activity. A burst is the sudden appearance of electrical impulses. The different types of seizures are caused by different
