Mabel Young was coming home from her weekly shopping trip. The bus was crowded but she managed to get a seat in the lower
saloon. Just before her stop she stood up to let a boarding passenger sit down. While she was standing she tripped over her own
shopping trolley. She tried to grab hold of the hand rails but she got her hand stuck. She twisted her left wrist in the fall and also
cut her forehead when it hit the seat in front of her. Mabel was the only person injured in the incident. Although the cut to her
forehead looked bad, Mabel appeared to be more concerned about getting home as it was her 70th birthday today and she was
expecting a visit from some of her friends. She lived at number 6 Church Road, which is in Croydon, Surrey.
The driver stopped and asked Mrs Young if she required an ambulance. She didn’t want one at first but some other passengers
convinced her that she did. The driver made a emergency call and shortly afterwards the ambulance arrived and Mabel was taken
to King’s College Hospital.

The Bus Incident Report
Some Tips For Completing The Sketch Correctly
The sketch can be the most important part of the Incident Report. Remember a picture tells a
1000 words.
It is also the most common feature of the Incident Report to be inadequately filled out.
Filling this section out incorrectly could lead to you giving the wrong impression of what actually
happened. In some cases you may even imply you were at fault when actually you were not.

Some Tips For Completing The Sketch Correctly
Use a pen. Pencil may seem a better option as you can easily correct a mistake. But the fact that
the sketch can be so easily altered makes using a pencil unacceptable.
Complete the sketch first. Drawing the sketch first can save a lot of time. If you do make a
mistake you can start again on a new report without having to fill out the first three pages again!

Some Tips For Completing The Description Correctly
Write facts, not opinions.
“The other driver was drunk” is an opinion,
“I smelt alcohol on the other driver’s breath” is a fact.
Don’t write unnecessary information on the form. Information contained elsewhere on the report often isn’t needed in the
Incident Description section.
Write down all details that may be relevant. Make sure you include what other vehicles were doing and the colour of traffic
