In order to receive the 100% secretary’s award, each Club Secretary who has qualified MUST fill out the form and have it approved and signed by the Club President and one of the proper District Officers. The completed form must be mailed to the District Governor before August 1, 2007. __________________

____ 2. All MAR** postmarked or sent electronically by the 3rd day of the following month. ____ 3. Pay International and District per capita fees within 30 days of billing. ____ 4. Mail PU-lO1 and supplements before May 31, 2007. ____ 5. Attend the District Leadership Training School. ____ 6. Meet three (3) of the following six (6) requirements. a. Attend all three (3) Zone Meetings in your zone. b. Attend the Fall Conference or the USA/Canada Forum. c. Attend the District, State or International Convention. d. Sponsor at least one (1) new member. e. Have 100% attendance for the year. f. Participate in at least two (2) Inter-Club Visitation. The completed 100% Secretary’s Application Form, which is available from any District Officer or on the web site, must be validated with the following signatures:
