If you can’t afford an attorney and need help filling out forms for
your uncontested divorce, criminal record expungement, child
support application or modifcation, or small claims case, the Ken-
tucky Online Self-Help Assistant (KOSHA) may be able to help you.
KOSHA makes it easy for you to complete and file court forms
step 1: get started
Type www.kyjustice.org into the address bar as seen below, and
then hit enter.
You will then see a page that looks similar to this:

step 2: Select your program
On the right-hand side of the page, you will see an option to access the un-
contested divorce, expungement, or small claim forms. Click on the name of
the forms you would like to use.
step 3: begin your interview
Begin answering the questions you see on the screen. Follow the directions
Step 4: print your forms
Click “finished” when you are done answering the questions. The forms you
need to file with the court will be printed for you.
Step 5: File your forms
Follow the instructions on the website. Take your completed forms and
fees to your Circuit Court Clerk for filing.
