                                          BIRTH CERTIFICATE APPLICATION



Criteria for obtaining a birth certificate:

    •     We only have Records of Birth for births that occurred in Washoe County.
    •     We charge $13.00 per certificate. Certificates are original documents with a seal.
    •     When no father is listed on the birth certificate only the following are allowed to purchase a copy:
                    The mother of the person named on the certificate.
                    The person named on the certificate.
                    A court appointed legal guardian of the person named on the certificate.
    •     If there is a father listed on the certificate the following are allowed to purchase a copy:
                    The person named on the certificate.
                    The mother, father, grandmother, grandfather, sibling, court appointed legal guardian or spouse
                       of the person named on the certificate.
    •     If you are the legal guardian, you must have the court appointed guardianship papers with you.
          (Court papers must be in original form and have a court seal on them.)
    •     If you are the step-parent you must have court appointed legal guardianship papers with you.
    •     If a birth record is not found after searching, an $8.00 search fee will be retained with the balance
          refunded. However, no refunds will be issued at the window. All refunds will be mailed out within 4 to 6
          weeks of receipt.
