      APPLICATION FOR, AND ISSUANCE OF, THE REQUIRED
    ELECTRICAL, MECHANICAL AND PLUMBING PERMIT TO A
   PROPERLY LICENSED CONTRACTOR OR AN OWNER/BUILDER
 SHALL BE ACCOMPLISHED PRIOR TO THE ISSUANCE OF A MOBILE
           HOME PERMIT FOR A REPLACEMENT UNIT.

 NEW MOBILE HOME ELECTRICAL & MECHANICAL PERMITS MAY
  ONLY BE ISSUED TO PROPERLY LICENSED CONTRACTORS AND
 SHALL BE ACCOMPLISHED PRIOR TO THE ISSUANCE OF A MOBILE
                      HOME PERMIT.

THIS MAY BE ACCOMPLISHED IN CONJUNCTION WITH THE MOBILE
 HOME PERMIT AND THE PLUMBING PERMIT MAY BE ISSUED TO

 BY THE BUILDING DEPARTMENT THE ELECTRICAL SERVICE WILL
 BE ORDERED DISCONNECTED IN ACCORDANCE WITH SECTION 111
     (SERVICE UTILITIES) OF THE FLORIDA BUILDING CODE AS

ALLOWED BY THE NASSAU COUNTY BUILDING
DEPARTMENT.

                          MOBILE HOME REQUIREMENTS
                     2007 FLORIDA BUILDING CODE IN EFFECT

   Installer is required to install State Certification Label per f.s. 320.827 on mobile home HUD
   sticker/permanent plate. Up to a $1,000.00 State fine if found in violation per f.s. 320.831.

   Mobile Home to be unlocked to inspect four-wire connection in electrical panel for isolated neutral.

   Mobile Home Manufacturers set-up manual to verify proper set-up and installation to be at site.

   Data plate for wind zone requirements must indicate wind zone II or III in the State of Florida. (Wind
   zone I is not allowed within the State of Florida.)

   Water lines installed and insulated at all crossovers.

   Electrical Power Pole installed per Nassau County requirements – Underground wiring in conduit at
   18” depth Direct burial at 24” in depth. (Uncovered for required inspection)

   Grade under Mobile Home level and graded to direct water away from unit.

   Anchor requirements – 3150 lb. To 4725 lb. Uplift 48” min. – 4000 lb. To 6000 lb. Uplift 57”min. (Per
   Department of Highway Safety & Motor Vehicles –Mobile Home installation standards –Section
   15C-1.0104-Revision 406) Anchors must be flush with and not extended above grade.

   Stabilizer plates required on all tie downs, except over the roof tie downs.

   Entry stairs and landings at main doorway to comply with FBC Code, Section 1009.


                 ADDITIONAL REQUIREMENTS FOR MOBILE HOMES INSTALLED IN
                              SPECIAL FLOOD HAZARD AREAS


   Mobile home installed at 1 foot above established Base Flood Elevation (BFE)
   (1 ft. Freeboard)

   Exterior mounted equipment (AC Compressor – Well Pump) elevated to BFE by use of decay
   resistant platform and must be properly secured.

   Ductwork and plenum must be weather tight and installed per Florida Building Code, Section 603.13,
   603.14, 603.12 and local Flood Ordinance 2002-49.

B. Plans:
1. Blocking plan must be submitted with application and a blocking plan must be at the site for
    all required inspections..

2. If conventional foundation is used – A separate foundation permit is required and plans or
   drawings must be submitted, complete on a minimum size sheet of
   8-1/2” x 11”, either dimensioned or to scale with sufficient detail, fastening schedules and
   elevations of the structure, that are necessary to determine compliance with all local, state
   and federal requirements (Section 106 of the 2007 Florida Building Code, as amended and
   adopted by Nassau County, Florida) Plans or drawings must be signed and sealed by the
   appropriate design professional showing actual configuration and contain the following
   information:

    a. The cover or first sheet must include the following information:
       (1). The name of the project, owner and or address.
       (2). Flood Zone information including the flood zone (A, V, X, etc.) the base
            flood elevation (BFE), the proposed elevation of the first habitable floor or
            the lowest horizontal member in “V” Zones, the proposed elevation of the
            ground slab if utilized, and the proposed elevation of the grade immediately
            adjacent to the structure.
       (3). Proposed front, side & rear setbacks from property lines. (New Construction
           & Exterior Remodels and Additions)
       (4). Distances between and existing and/or proposed primary and accessory
            structures.

                       NORMAL PERMIT APPLICATION PROCESS

1. Application for permit is checked for completeness.
   (Application returned to applicant if incomplete)
2. Plans and/or Site Drawings are reviewed for compliance with appropriate Zoning regulations
3. Plans and/or Drawings are reviewed for code compliance with the technical codes.
   a. (minor code violations are corrected by the reviewer utilizing “Shall Comply” on
        the Plan or Drawing)
   b. (The Plans or Drawings are returned to the applicant with a “Plans Review for
        Code Compliance” form for correction by the applicant for other than minor
         code violations)
4. All fees are assessed. The cost of a mobile home is determined by the size of the mobile
   home, impact fees and zoning fees.

      MOBILE HOME INSTALLATION PERMITS
                         October 1, 2001 (Revised July 27, 2004)
The following policy and procedures are to be followed in compliance with the
requirements of Section 320.8249, Florida Statutes and the Florida Administrative
Code, Rule 15C-2.0073(1). The process for permitting fees associated with this
compliance has also changed due to licensing requirements contained in Chapter
489 of the Florida Statutes and are noted in the text of this document.

   1. A permit to install and tie down a mobile home shall only be issued to an
      individual or company licensed by the State of Florida as required by Florida
      State Statute Section 320.8249.
         a. A permit to install and tie down a mobile home cannot be issued as an
             Owner/Builder as allowed under the exemption of Chapter 489,
             Florida Statutes.

   2. A separate permit is required to supply any electrical service, plumbing
      installation or mechanical equipment and must be applied for and issued to a
      properly licensed individual or company.
          a. A permit to install electrical service, plumbing installation or
              mechanical equipment for a mobile home cannot be issued to an
              Owner/Builder under the exemption of Chapter 489, Florida Statute.
   Exceptions:
   a. A permit to up-grade, repair, replace or re-install the electrical service,
      plumbing hook-up or mechanical equipment on an existing mobile home.
   b. A permit to hook-up the existing electrical service, the plumbing or the
      mechanical equipment to a “re-placement mobile home if the replacement
      mobile is located in the exact same location as the replaced unit with no
      relocation or extension of service wiring, piping or mechanical feed lines or
      wiring.

   3. The fees associated with the actual mobile home installation have been
      reduced to reflect the additional fees associated with the permits required for
      the electrical, plumbing and mechanical installations.

   4. The inspections required are as follows:
         a. Mobile home installation requires a blocking and tie down inspection
             30 days after permit issuance and prior to installation of any skirting.
         b. Electrical permit requires a rough inspection after the trench is dug
             for an underground installation and before the raceway is covered. A
             meter set inspection after all connections, grounding and disconnects
             have been installed. Overhead service requires a meter set inspection
             after all connections, grounding and disconnects have been installed.
             All permits require a final inspection after all work is complete and

              before occupancy occurs. All inspections referenced may be done at
              the same time and may also be done in concurrence with the Tie-
              Down, Plumbing and Mechanical Inspections.
           c. Plumbing permits require a rough inspection after the drain line and
              cleanout is installed to the sanitary sewer or septic system and before
              the piping is covered. The potable water requires a rough inspection
              after the piping is installed from the well or water purveyor and
              before the piping is covered up (Drain & Potable Inspections can be
              done at the same time) All permits require a final inspection after all
              work is complete and before occupancy occurs. The Inspection may
              be done in concurrence with the Tie-Down, Electrical and Mechanical
              Inspection.
           d. Mechanical permits require a final inspection to determine the
              following:
              (1). The unit is attached to a proper base.
              (2). The unit is at the proper elevation if located in a Flood Zone.
              (3). The unit has a required disconnect.
              This inspection may be done with the Tie-Down, Electrical and
              Plumbing Inspection.

    5. After February 1, 2002, all mobile home installations will be required to have
       code compliant entrance stairs and landings prior to issuance of Certificate
       of Occupancy.
    6. Effective on October 1, 2001, all mobile homes will require the following
       permits:
           a. A Blocking and Tie Down Permit issued to a Licensed Mobile Home
              Installer at the fee indicated in “Attachment A”.
           b. An Electrical Permit issued to a Licensed Electrical Contractor at the
              fee indicated in “Attachment A”. (See Exceptions Number 2)
           c. A Mechanical Permit issued to a Licensed Mechanical Contractor at
              the fee indicated in “Attachment A”. (Note: A Mechanical Permit is
              not required for units that have a self contained unitized system) (See
              Exceptions Number 2)
           d. A Plumbing Permit for sewer and potable water issued to:
              (1). A Licensed Mobile Home Installer if the only connection is to the
              Septic System or a Sanitary Sewer Tap that has been installed by
              (a). Properly Licensed Contractor and to a water meter or well that
                   has been installed by a properly licensed contractor. (See
                   Exceptions Number 2)
              (2). A State of Florida Plumbing Contractor Licensed under Florida
                   Statute Chapter 489. (See Exceptions Number 2)

COMMENTARY:

The Licensed Mobile Home Installer may perform all Mobile Home electrical
interconnections between modular units although the Electrical Service Installation

on a new mobile home placed on a vacant property may only be performed by a
State of Florida Licensed Electrical Contractor.

All Foundation Systems other than those approved in 15C-1 shall be submitted with
separate plans that are signed and sealed by a design professional registered by the
State of Florida and shall receive a separate permit after submission of associated
fees.
                               Florida Mobile Home Installer Statutes
320.8249 Mobile home installer license –

   (1) Any person who engages in mobile home installation shall obtain a mobile home installer’s license from
       the Bureau of Mobile Home and Recreational Vehicle Construction of the Department of Highway
       Safety and Motor Vehicles pursuant to this section. Said license shall be renewed annually and each
       licensee shall pay a fee of $150.

   (2) The Department of Highway Safety and Motor Vehicles shall issue a license as a mobile home installer
       to any person who applies to the department, pays the appropriate application fee, not to exceed $100, as
       set by department rule, and complies with subsection (3).

   (3) In order to obtain licensure as a mobile home installer, the applicant must be at least 18 years old, must
       hold a valid performance bond in an amount set by department rule, not to exceed $5,000 conditioned
       upon proper performance of mobile home installation and weather-sealing duties for a period of 1 year,
       must carry liability insurance in an amount determined by department rule, not to exceed $100,000 must
       complete a minimum 8-hour training course approved by the department, and must pass a department
       -approved examination designed to test the skills necessary to properly and competently perform mobile
       home installation and to ascertain that the applicant has adequate knowledge of federal, state, and
       local laws applicable to mobile home installation contracting. The department may charge an
       examination fee sufficient to defray the cost of developing or obtaining and providing the examination,
       not to exceed $100. Any licensed dealer or licensed manufacturer who has subcontracted with an
       installer for installation and who remedies any faulty installation performed by said installer shall have
       recourse against said installer’s performance bond.

   (4) Notwithstanding the provisions of subsection (3), any person who can show that he or she had been
       engaged in the business of mobile home installation on October 1, 1996, shall be exempted until
       October 1, 1997, from the requirement for completing training and for passing an examination in
       order to be licensed by the department as a mobile home installer and shall be licensed upon
       application, provided he or she has complied with all requirements of subsection (3), other than the
       training and examination requirements. No person shall be licensed or remain licensed as a mobile
       home installer subsequent to October 1, 1997, who has not taken and passed the department-
       approved mobile home installer examination.

   (5) A direct employee of a licensed mobile home installer working under the supervision of the licensee
       and within the job scope of the licensee is not required to be licensed as a mobile home installer.
       The licensed mobile home installer is responsible for supervising all such employees and for the
       proper and competent performance of all employees working under his or her supervision.

   (6) “Installation” as used herein, is synonymous with “setup” as defined in s. 320.822(14).
(7) No person shall:
       (a) Falsely hold himself or herself or a business organization out as a licensed mobile home installer.
       (b) Falsely impersonate a licensed mobile home installer.
       (c) Present as his or her own the mobile home installers license of another.
       (d) Knowingly give false or forged evidence to the department.
       (e) Use or attempt to use a mobile home installers license which has been suspended or revoked: or
       (f) Engage in the business or act in the capacity of a licensed mobile home installer or advertise
           himself or herself or a business organization as available to engage in the business or act in the
           capacity of a mobile home installer without being duly licensed.

(8) Any unlicensed person who violates any of the provisions of subsection (7) is guilty of a misdemeanor
    of the first degree, punishable as provided in s.775.082 or s.775.083.

(9) No licensed person nor licensed applicant shall:
       (a) Obtain a mobile home installers license by fraud or misrepresentation.
       (b) Be convicted or found guilty of, or enter a plea of nolo contendere to, regardless of adjudication,
           a crime in any jurisdiction which directly relates to the practice of mobile home installation or
           the ability to practice.
       (c) Violate any lawful order of the department.
       (d) Commit fraud or deceit in the practice of contracting.
       (e) Commit incompetence or misconduct in the practice of contracting.
       (f) Commit gross negligence, repeated negligence, or negligence resulting in a significant danger to
           life or property.
       (g) Commit violations of the installation standards for mobile homes or manufactured homes
           contained in rules 15C-1.0102 to 15C-1.0104, Florida Administrative Code.

  (10)Any licensed person or license applicant who violates any provision of subsection (9) may have any
      of the following disciplinary penalties imposed by the department.
          (a) License revocation.
          (b) License suspension.
          (c) A fine not to exceed $1,000 per violation.
          (d) A requirement to take and pass or retake and pass the department-approved examination.
          (e) Probation.
          (f) Probation subject to such restriction of practice as the department chooses to impose.
          (g) A notice of noncompliance; or
          (h) Refusal of licensure application.

(11) Licensed mobile home dealers and licensed mobile home manufacturers are exempt from requirements
 to obtain a license as a mobile home installer and may perform mobile home installation. Any licensed
 dealer or licensed manufacturer who does not subcontract with a licensed installer and who performs his
 or her own installations, either himself or herself or through direct employees, shall have at least one
 employee who has completed an 8-hour installation training course, as approved by the department.
 Licensed mobile home dealers and mobile home manufacturers are subject to discipline against their
 license for violation of subsection (9).
(12) The regulation of manufactured home installers or mobile home installers is preempted to the state,
      and no person may perform mobile home installation unless licensed pursuant to this section, regardless
      of whether that person holds a local license.

  (13) No county, municipality, or other unit of local government may require additional licensing of a duly
       licensed installer who performs setup operations as defined in s.320.822. However a county,
       municipality, or other unit of local government may require an installer to obtain a local occupational
       license, which license shall not require for its issuance any conditions other than those required by
       this act and payment of the appropriate occupational license fee.

(14) All installers, dealers and manufacturers shall purchase installation decals from the Department of
     Highway Safety and Motor Vehicles for a fee not to exceed $10 per decal. An installation
     decal shall be affixed to the manufactured home or mobile home prior to installation. This
     decal shall denote the date of installation, the name of the installer and the number of installer’s
     license or the dealer or manufacturer license number. Such decal shall be positioned
      immediately next to the HUD decal.

(15) In performing the installation, installers shall not perform plumbing or electrical activities prohibited by
     department rules related to setup operations pursuant to s.320.822.

(16) Funds received by the department pursuant to this section shall be deposited in the Highway Safety
     Operating Trust Fund.

(17) There are hereby appropriated five positions and $219,295 from the Highway Safety Operating Trust
     Fund in the Department of Highway Safety and Motor Vehicles to implement the provisions of this
     section.

                        SERVICE GENERAL INFORMATION


   1. 48 INCH LONG (HELIX) Auger anchor approximately 5’ – 10’ from pole.
   2. Guy wire ¼ inch diameter minimum with cable clamps. (tape cut end) opposite service
       drop direction.
   3. Three insulated Conductors, unless part of a SE cable. Identify Neutral, with White tape
       or paint 02 NEC 230-22. Conductor clearance 12’ above finish grade. 15’above
       driveway, 18’ above roads. 02 NEC 230-24(Provide 24” – 36” free conductor out of
       weather head).
   4. “Wheather head” and riser, sized by service size (200 AMP service = 2” conduit)
   5. Eye bolt 5/8 inch diameter for service drop and guy wire.
   6. Pressure treated pole approximately 5 inches diameter at small end. 20 or 25 foot in
       length Required burial depth, 4’ for 20’ pole and 5’ for 25’ pole.

   2. “Ground wire” Run #4 copper grounding electrode conductor from the two
       grounds rods (unspliced) to and terminate at the neutral lug in meter base. Staple
       ground wire to post. 02 NEC 250-66, 250-24 and 250.94.
   3. Four insulated, color coded conductors to mobile home panel. Isolate neutral in
       mobile home panel 24” direct buried or 18” if conductors are in conduit
       02 NEC 300-5, 550-24.
   4. Ground clamp solid bronze or approved for direct bury “acorn clamp” or equal
       02 NEC 250-70.

 ALL INSPECTION REQUESTS ARE REQUIRED TO BE INITIATED BY THE PERMIT HOLDER,
  ALL INSPECTION REQUESTS SHALL SUPPLY THE PERMIT NUMBER FOR EACH TRADE
  REQUESTED, THE TYPE OF INSPECTION, THE NAME OF THE INDIVIDUAL MAKING THE
  REQUEST AND A TELEPHONE NUMBER WHERE THE INDIVIDUAL CAN BE REACHED IF
                                  NECESSARY.

THE FOLLOWING IS A LIST OF THE MOBILE HOME INSPECTIONS PERFORMED BY THE
NASSAU COUNTY BUILDING DEPARTMENT WITH A BRIEF DESCRIPTION OF THE PURPOSE FOR
                                THE INSPECTION

INSPECTIONS REQUESTED FOR THE FOLLOWING WORK DAY MUST BE CALLED IN PRIOR
                        TO 4:00 P.M. THE DAY BEFORE.

TYPE                               DESCRIPTION
000 - CERTIFICATE OF OCCUPANCY INSPECTON: This inspection is made after all final
      inspections have been approved and allows the issuance of a Certificate of Occupancy to be
      issued for a habitable structure or area.

100 – FOOTER/FOUNDATION: This inspection is made after a foundation or footing has been dug
      and all required re-inforcement or inbedded items have been placed and before any concrete
      has been placed.

125- PRE-POUR: This inspection is made on anything that is going to have concrete poured that is not
     covered by another inspection type such as filled cells, stem wall, patios, lift stations, etc. and is
     made before the concrete is placed

190 - MOBILE HOME TIE DOWN- This inspection is made after any wind load related and
      required strapping or attachments are in place and before skirting is installed around the
      perimeter of mobile home and before the final mobile home inspection. Permit will be void
      if this inspection is not completed within 30 days of permit issuance.

210 – ELECTRICAL TEMPORARY: This inspection is made after the temporary pole or temporary
      installation is complete and before the meter is set and power connected.

260 – ELECTRIC METER SET: This inspection is made after all electrical components are in a safe
      condition with all required grounding in place and is utilized to have electrical power for the
      purpose of hot checking appliances and circuits. (This is NOT a final electrical inspection)

288 – DISCONNECTED SERVICE INSPECT: This inspection is made when the electrical service
      has been disconnected for any reason and the applicant is requesting a reconnect. (This
      inspection is used to determine if the system is still safe for reconnection of power)

299- ELECTRICAL FINAL/CHANGE OUT: This inspection is made after all components, fixtures,
     equipment and appliances indicated on the approved construction drawings have been installed
     and the electrical system is in a complete, safe and code compliant state and ready to be used.


350 – PLUMBING ROUGH/GROUND: This inspection is made after all plumbing piping has been
      installed under the slab and before concrete is poured and is not covered up with dirt over any
      connections and has the required head of water on drainage piping or the required pressure of
      approximately 80 to 100 PSI on water piping. (This inspection is utilized to determine if there
      are any leaks in the system and if the proper materials were utilized)

399 – PLUMBING FINAL: This inspection is made after all plumbing fixtures have been installed,
      are operational and the system is ready for its intended use.

450 – MECHANICAL ROUGH: This inspection is made after the air conditioning or hood exhaust
      ductwork has been installed, and prior to being covered up with any construction material and is
      utilized to ascertain that the installation has been properly supported and meets the code
      requirements.

499 – MECHANICAL FINAL: This inspection is made after all components of a mechanical system
      are installed are operational and ready for its intended use.

600 – INFORMATIONAL VISIT: This inspection is made when a citizen or contractor is unsure of a
      requirement or needs assistance with the project and is at the discretion of the Building
     Official or the Building Inspector.


999 – FINAL INSPECTION: This inspection is a required inspection on all permits issued including
      demolition permits and is utilized to insure that all aspects of the project or individual permit
      have been completed, meet all applicable codes and ordinances and that the building,
      installation or demolition is complete and ready for its intended use.




       FINAL INSPECTIONS ARE REQUIRED ON ALL PERMITS.
      FAILURE TO OBTAIN FINAL INSPECTION MAY RESULT IN
