(1) A fetal mortality rate used in the Healthy People 2010 Objectives is the number of fetal deaths with gestation of 20 weeks or more
per 1,000 live births plus fetal deaths.

(2) Definition of maternal death used in the Healthy People 2010 Objectives is the death of a woman while pregnant or within 42 days
of termination of pregnancy from any cause related to or aggravated by the pregnancy or its management, but not from accidental or
incidental causes. International Classification Disease – The Tenth Revision (ICD–10) codes include A34, O00–O95, and O98–O99.

(3) Only include diseases that are declared reportable with reporting requirements (Louisiana Public Health Sanitary Code):
Chancroid, Chlamydia, Gonorrhea, Syphilis, and Lymphogranuloma Venereum.

(4) Kotelchuck index is used to measure early and adequacy prenatal care (PNC).
    - Adequate plus PNC: PNC initiation begun in month 1,2,3, or 4 and 110 % or more expected PNC visits were received

    - Adequate PNC: PNC initiation begun in month 1,2,3, or 4 and 80% to 109% of expected PNC visits were received
    - Intermediate PNC: PNC initiation begun in month 1,2,3, or 4 and 50% to 79% of expected PNC visits were received
    - Inadequate PNC: PNC initiation begun in month 5 or late or less than 50% of expected PNC visits were received
Early and adequate PNC includes adequate plus PNC and adequate PNC.

(5) Include obstetrical level III and level III regional units.

(6) A pregnant woman who has full term (37-42 weeks), singleton, and vertex presentation baby.
