                           Quarterly Report Template
*** This quarterly report template is a MICROSOFT WORD FORM. This means that this form is locked
and you can only type in the gray text fields or click on checkboxes.

                                         Grey text fields:

1. In the Demographics section, please fill in all information requested.
2. In the Measures section, indicate when (e.g. Tuesday at 10:00 am) and how (e.g. by consulting the
   scheduler) each of the measures is collected, and who is responsible for collecting that data.
3. In the Access, Office Efficiency and Clinical Care sections type in your report into the appropriate
   box for the quarter you are reporting in. There are 3 questions to answer for each quarter:


               Q1 (Quarter 1)      January – March (report due in March)

               Q2 (Quarter 2)      April – June (report due June)

               Q3 (Quarter 3)      July – September (report due September)

               Q4 (Quarter 4)      October – December (report due December)

4. In the Comments section, please add any additional information you would like that further describes
   your clinic, your activities, and the improvements you are making.
5. Your quarterly report will develop like a journal or diary. Please use the most recent report containing
   feedback from faculty.

reference while completing this report. Your facilitator is a resource
available to assist you in this process.
