This injury will be classified as a First Aid. 1/20/2005 - Employee sustained throat irritation when a sample cylinder PRV valve "popped off", spraying the employee with light hydrocarbon.This injury will be classified as a First Aid. 1/25/2005 - Employee sustained a cut to the right hand from a nail sticking out from the wall. This injury will be classified as a First Aid. 1/26/2005 - Employee pinched his ring finger on his right hand with a pair of pliers. This injury will be classified as a First Aid. 1/30/2005 - Employee sustained a steam burn to the wrist, while opening a n exhaust steam valve. This injury will be classified as a First Aid. 1/31/2005 - Employee slipped on oily steps bruising left hip and leg. This injury will be classified as a First Aid. 2/15/05 - Employee reported work as a master craft resulted in sympthons of carpet tunnel. This injury will be classified as a First Aid at this time. 2/24/2005 - A Motiva employee sustained a puncture wound from a metal shard embedded in the right index finger. This injury will be classified as a First Aid. 2/28/2005 - A Motiva employee sustained a strained right knee while walking. This injury will be classified as a First Aid. 3/21/2005 - A Motiva employee sustained a strained back pulling on a loading arm. This injury will be classified as a First Aid. 3/28/2005 - A Motiva employee sustained a twisted knee walking through mud. This injury will be classified as a First Aid. 4/6/2005 - A Motiva employee sustained a mashed hand, when wrench slipped while openning valve. This injury will be classified as a First Aid. 4/12/2005 - A Motiva employee sustained a bruised forearm, when the employee slipped on a staircase. This injury will be classified as a First Aid. 4/13/2005 - A cat scratched a Motiva employee causing a laceration to the hand. This injury will be classified as a First Aid. Motiva Recordables Injuries: 02/10/2005 - A Motiva employee sustained a OSHA recordable injury when a battery box exploded striking the employee in the left hand. The incident caused several lacerations and broken bones. This injury will be classified as a Recordable and Lost Time injury 2/11/05 - Employee states he went to step down from one level of scaffold and his foot slipped causing him to fall, striking the back of his head and injuring his back.
