           This letter is to follow up on your application, on-line course materials and timed
    course outline that I received on November 2, 2009 requesting approval of an InterNACHI
    sponsored on-line training entitled “Wood-Destroying Organism Online Course: Inspection
    and Control Training for Inspectors and Treatment Applicators” that is being made available
    Nevada Pesticide Continuing Education Units. Page two and page three outline some
    additional information related to this approval.
           Based on information on your application and supporting course documentation, I have
    approved the InterNACHI – Wood-Destroying Organism Online Course: Inspection and
    Control Training for Inspectors and Treatment Applicators training for 12.0 total continuing
    education units (CEU’s) or credits for Nevada. Based on Nevada’s approval criteria, this
    conference qualifies for 10 General and 2 Laws CEU’s.
            This course is approved for three (3) years and will expire on December 31, 2012
    unless withdrawn for good cause at an earlier date. Your Nevada Course Number = NV-09-
    076. This course number must be placed on all attendance sheets that you will submit to the
    State of Nevada at the completion of an applicators on-line training.

            Certificate of Completion: Please be reminded that if an attendee requests an
    certificate of completion, the course provider is required to provide such a certificate (refer to
    page three of the 2009 - 2010 Nevada Pesticide Applicators Continuing Education Course
    Approval Guide for required information).
            Attendance Recording: I have not enclosed any Nevada sign-in sheet for verification
    for attendance for this on-line training. I will accept an alternate method of attendance
    verification as long as, it meets the requirements in NAC 555.374.4. Please be reminded that
    some kind of Positive ID is required to be presented by the attendee at the time attendance is

For Nevada Custom Pesticide Applicators:
  !   This training is for educational purposes only (Pesticide Continuing Education
      Units (CEU’s)) and should not be used for examination purposes.
  !   Upon completion of this training, a Pesticide Applicator will earn 2 Law CEU’s
      and 10 General CEU’s for a total of 12 CEU’s. All Chapters must be completed
      and a passing grade earned on all quizzes and final exam before CEU’s will be
      earned.
  !   Chapter 21: Laws Concerning WDI … In this chapter the Ohio Department of
      Agriculture is used as an example. So when Category 10b is referenced this
      would be category C4 in Nevada. Nevada Wood-Destroying Pest inspection
      Reporting Regulations can be found at in NAC 555.430

  !   Chapter 22: Training and Licensing Requirements … The information
      presented greatly differs from Nevada’s licensing requirements. The Nevada
      Department of Agriculture suggests that you refer to NAC 555.270 thru NAC
      555.397 (http://www.leg.state.nv.us/NAC/NAC-555.html#NAC555Sec270) for
      Nevada specific applicator licensing requirements (examinations), business
      licensing, insurance requirements, license renewal requirements and fees.
  !   Chapters 23 (The Real Estate Transaction) and 24 (Guidelines for Completing
      the NPMA-33 Form). In both chapters the NPMA-33 form is referenced.
      Nevada DOES NOT ACCEPT this form for any real estate transactions in the
      state. The State of Nevada has developed their own form entitled “Wood-
      Destroying Pest Inspection Report” which is required to be completed (this is
      pointed in Chapter 23). The Nevada Department of Agriculture suggests that
      you become familiar with correspondences with HUD on this issue. The
      NDOA has made these correspondences available on their web site at:

For Nevada Certified Applicators (Commercial or Private
Applicators):
  !   THIS TRAINING DOES NOT QUALIFY YOU TO DO ANY KIND OF WOOD-
      DESTROYING INSPECTIONS. ONLY LICENSED NEVADA CUSTOM
      APPLICATORS CAN CONDUCT INSPECTIONS TO BUILDINGS. DOING
      SUCH INSPECTIONS OR COMMENTING ON ANY WOOD-DESTROYING
      ORGANISM WILL MAKE YOU IN VIOLATION OF NEVADA REVISED
      STATUTE (NRS) 555.285 (http://www.leg.state.nv.us/nrs/NRS-
      555.html#NRS555Sec285).
  !   This training is for educational purposes only (Pesticide Continuing Education
      Units (CEU’s)).
  !   Upon completion of this training, a Pesticide Applicator will earn 2 Law CEU’s
      and 10 General CEU’s for a total of 12 CEU’s. All Chapters must be completed

      certification renewal requirements and fees. For certification; business
      licensing and liability insurance is not required.
  !   Chapters 23 (The Real Estate Transaction) and 24 (Guidelines for Completing
