        Category 1: Mandatory inclusions

        Personal safety checklist; to be completed within 3 months of starting

        DOPS forms : all nine (9) general microbiology benches to be completed before
        sitting Part I. Special benches may be completed before or after Part I.

        CbD forms: from five (5) different sites of infection before Part I. An additional four
        (4) high complexity case to be completed before sitting Part II.

        Incident reports, reflections on significant events – 2 per year
        Supervisor's report/s for each year and/or rotation.



        Category 2: Clinical meetings                                             (minimum 3
        different activities per year).
         Record ONLY clinical meetings here. Meetings concerned with management QA, ethic
        Multidisciplinary clinical meetings, grand rounds, ward rounds, clinico-pathological
        correlation (CPC), morbidity and mortality (M&M) meetings, etc.


        Conferences, courses, seminars, workshops, forums

        Category 3: Research/Scholarship                                         (minimum 3

        Literature review and preparation of materials (eg plates, photograpgs, Gram or

organisms from laboratory database, using CLSI (Clinical and Laboratory
Starndards Institute) or other appropriate guidelines including rationale for selection
and reporting of these
