Using the Customer Satisfaction Survey template
When gathering important information from your customers, this customizable template can
become a critical tool to facilitate the collection, organization, and ongoing use of customer-
survey data. The information that follows will help you edit and use this template.
CUSTOMIZE A FORM
To add your company name and logo, or to make any changes to the layout of the form, open it in Design mode.
To do this, on the File menu click Open in Design Mode and then select and open the form to customize.
To add your logo to the form, select and delete the place-holder logo graphic. On the Insert menu, point
to Picture and then click From File. Select your logo picture file and click Insert.
To resize your logo, select the picture. On the Format menu, click Picture. On the dialog, on the Size tab,
type the new dimensions and click OK.
To edit the questions in this form, click in the text of a question and edit it as you would in any electronic
When working in Design Mode, tools are available to edit the layout and the form controls, such as text boxes and
date pickers. To access these tools, on the View menu click Design Tasks. A task pane opens. You can then access
layout, controls, and other design elements such as the source data.
PUBLISH YOUR FORM FOR OTHERS TO USE
When your form is ready to be shared with your team, you can save the customized version and then publish it in a
shared network location, or in a SharePoint site. With a SharePoint site, you can publish it as a template in a new
document library, or publish it as a content type available to multiple document libraries.
If you publish the form in a document library, you can specify fields, in the form, as column headings that appear in
the library view. In addition, users with access to that library can click the Fill Out This Form option to open a new
instance in Microsoft Office InfoPath 2007.
To publish a form, open it in Design Mode. On the File menu, click Publish. The Publishing Wizard appears, and
with it you can publish the template in a network location, or a SharePoint site.
FILL OUT A FORM
To fill out a form, click in any field, then type information, or you can select an option from a list, if provided.
For fields that require a date, you can type the date, or use the Date Picker icon, a small calendar, to the right of
the filed. Click the icon to select a date from the dialog.
