1.0 RATIONALE
    This survey is the only consistent and reliable source of mineral exploration, deposit appraisal and mine complex development statistics for Canada. These statistics provide both industry and
    governments with the basis for monitoring exploration levels and for understanding the economics of the mineral development cycle in Canada. The accuracy of the survey depends on you.
    For guidance on how to complete the questionnaire, consult the Quick Index, Reporting Guide and Guidelines.

2.0 PROVINCE OR TERRITORY COVERED BY THIS REPORT
    Separate reports are required for each province or territory in which exploration, deposit appraisal or mine complex development activities occur. However, if you report for more than one
    province or territory on this form, please indicate so clearly. Include activities related to metals and nonmetals, but exclude activities related to oil and gas. For additional forms, contact the
    appropriate agency (see page 4).
