PRICING AND QUOTE WORKBOOK
CONSULTING SERVICES CONTRACT
INSTRUCTIONS

This "Pricing and Quote Workbook" is to be used by contractors when providing quotes to state agencies and
other public bodies for specific consulting projects covered under the contract services contract. Prior to
developing a quote, read all documents posted for this contract at www.eva.virginia.gov . This includes this
"Pricing and Quote Workbook", including the Instructions, Contract Pricing, Contractor Info, Approach, Past
Projects, Deliverables, Miscellanious, and Price Quote tabs at the bottom of the workbook; and 2) the "Notice of
Contract Award" document, including contract overview, instructions, contract procedures/requirements, and
terms and conditions.


When providing quotes, complete and provide the following six (6) forms (see tabs below) to the
requesting state agency or other public body:

3. Past Projects
4. Deliverables
5. Miscellanious
6. Price Quote

Deviation from the prescribed format is NOT encouraged as it makes it more difficult to review quotes.
The "Miscellanious" tab allows the contractor to provide supplemental information not covered on the other forms.

IMPORTANT: THE FOLLOWING TERM AND CONDITION APPLIES TO ALL QUOTES:

APPROVAL OF CONTRACTOR/SUBCONTRACTOR PERSONNEL: Personnel commitments identified in the
Contractor's quote to the requesting state agency or other public body shall be considered mandatory to the work
performed under the resulting contract. Staffing must include those individuals proposed. With the exception of
death or the removal of personnel who have permanently terminated employment or who become unavailable due
to an extended illness, no changes to personnel will be made by the Contractor or the Contractor's Subcontractors
without the prior written consent of the requesting state agency or other public body. Replacement of any
personnel, if approved, shall be with personnel of equal ability, experience and qualifications.

The Contractor's key personnel shall not be diverted from the project without prior written consent of the requesting
state agency or other public body. Key personnel are those individuals who are determined by the requesting state
agency or other public body to be central to the management of the project and to the project's
development/implementation. Changes or diversion of Contractor key personnel without prior written consent of the
requesting state agency or other public body will be sufficient grounds for termination of the contract for default.
If personnel are lost because of death, termination or extended illness, the Contractor will replace them within ten
(10) working days, including pre-approval of the replacement. The requesting state agency or other public body
may approve up to a ten (10) day working day extension for replacement of these personnel if the Contractor
submits a written request for an extension of time. The Contractor shall include in the required status reports its
efforts and progress in finding replacements and the effect of the absence on the progress of the project. In
addition, the Contractor shall make interim arrangements to assure that the project's progress is not adversely
affected by the loss of the personnel.

TRAVEL-RELATED EXPENSES: The contractor’s hourly prices include all costs except travel-related expenses. Hourly
prices include, but are not limited to, consulting services, salaries, fees, profit, overhead, administrative costs, copying,
working facilities and supplies.
A contractor’s or subcontractor’s actual travel-related expenses, including lodging, meals, and mileage, may be reimbursed
by the agency as appropriate if all of the following conditions are met:

1) Detailed descriptive and price information for each travel-related expense is provided in the contractor’s quote to the
agency prior to award of the order to the contractor by the agency, and travel-related expenses are approved by the agency
and included in the order to the contractor.
2) Travel-related expense reimbursements to contractors do not exceed the “State Travel Regulations” policies and limits
in the most current Commonwealth of Virginia, Department of Accounts “Commonwealth Accounting Policies and
Procedures (CAPP) Manual”. The CAPP Manual is available at the DOA website at http://www.doa.virginia.gov , and State
Travel Regulations can be found under Topic Number 20335. An agency may set more restrictive travel reimbursement
requirements than those found in the CAPP Manual.
3) The contractor’s hourly pricing rate will not be paid for the travel time of a contractor or subcontractor.

INSURANCE: By signing and submitting a bid or proposal under this solicitation, the bidder or offeror
certifies that if awarded the contract, it will have the following insurance coverage at the time the
contract is awarded. For construction contracts, if any subcontractors are involved, the subcontractor
will have workers' compensation insurance in accordance with §§ 2.2-4332 and 65.2-800 et seq. of
the Code of Virginia. The bidder or offeror further certifies that the contractor and any subcontractors
will maintain these insurance coverage during the entire term of the contract and that all insurance
coverage will be provided by insurance companies authorized to sell insurance in Virginia by the
Virginia State Corporation Commission.
MINIMUM INSURANCE COVERAGES AND LIMITS REQUIRED FOR MOST CONTRACTS:
1)Workers' Compensation - Statutory requirements and benefits. Coverage is compulsory for
employers of three or more employees, to include the employer. Contractors who fail to notify the
Commonwealth of increases in the number of employees that change their workers' compensation
requirements under the Code of Virginia during the course of the contract shall be in noncompliance
with the contract. 2). Employer's Liability - $100,000. 3) Commercial General Liability - $1,000,000 per
occurrence. Commercial General Liability is to include bodily injury and property damage, personal
injury and advertising injury, products and completed operations coverage. The Commonwealth of
Virginia must be named as an additional insured and so endorsed on the policy.

FUNCTIONAL RESPONSIBILITIES: Listed below are the functional responsibilities for each of
the five (5) contract job titles. Hourly prices under the contract will be paid for the hours a person
spends providing these functional responsibilites. For example, some firms may have a specific
individual for each job title and functional responsibility. This may mean that a specific individual
will always perform as, say, a senior consultant and be paid as such. Other firms may have one
employee who will provide all the functional responsibilities for a specific project. For example,
the same person may provide three (3) hours of work that meets the functional responsibilities of
the "Principal" job title, eight (8) hours of work that meets the functional responsibilities of the
"Consultant" job title, and four (4) hours of work that meets the functional responsibilities of the
"Program and Administrative Support" job title. This person would be paid three (3) hours at the
"Principal" hourly rate, eight (8) hours at the "Consultant" rate, and four (4) hours at the "Program
and Administrative Support" rate. Contractors will identify the
number of hours required for each job title/functional responsibility on this pricing sheet when
submitting a quote for a project. If a specific job title/functional responsibility is not required for
a particular project, put "NA" (for "not applicable"). The hourly pricing shown in the "Pricing
Schedule" are maximum prices....the contractor may offer hourly prices that are lower than those
listed, but not higher. The hourly rate paid shall be consistent with the functions that are
provided.

SPECIAL REQUIREMENTS: Examples of special requirements include increased insurance,
bonds or other forms of security that a state agency or other public body may require as part of
their scope of work. The cost of special requirements may be paid for by the state agency or
other public body, but only for the contractor's actual cost for providing the special requirements,
and only if the state agency or other public body agrees to the special requirements and related
cost by inclusion of such in the purchase order to the contractor. Contractor shall provide written
documentation of such costs to the agency, upon request.
