        Mayo Clinic is inviting you to participate in a survey focusing on pay practices for non-exempt positions typically found across organizations in Rochester,
        Minnesota.

        Mayo Clinic has a need to collect pay data for timely business decisions. This survey covers upcoming compensation adjustments, and pay practices for 25

2. Read the survey job descriptions in Appendix A.

3. Report pay data for survey positions using the Reporting Pay Data Form tab. Report positions only where a close correspondence exists between
your organization’s job and the survey job description. To match a position, almost all of your organization’s major job responsibilities, requirements,
and reporting relationships should be highly comparable to the survey job description.

            Instructions
              1. Review the survey jobs in Appendix A tab and report pay data for your positions which are a good match to survey positions.
              2. Insert the survey job code in the first column and indicate your organization’s job title in the second column.
              3. Indicate if the position is part of a bargaining unit (“Y” for “yes” and “N” for “no”).
              4. Indicate the FLSA status for each reported position.
                     a. “Non-Exempt” (NE) from overtime provisions (eligible for overtime).
                     a. “Exempt” (E) from overtime provisions (not eligible for overtime).
              5. When reporting hourly pay rates, report the pay rate for each individual employee in a position that matches a survey position. Optional: Report the average hourly pay rate for positions with
              multiple incumbents and indicate the number of employees in the position.

              6. If your organization has a formal pay structure, indicate the applicable hourly pay range (minimum, midpoint, maximum) for the position or union scale if the position is part of a bargaining
              unit.
