ORDERING INSTRUCTIONS Consolidated Pediatric/Vaccines For Children (VFC) Contracts Period: April 1, 2009 through March 31, 2010
The Centers for Disease Control & Prevention (CDC) has awarded five contracts for the purchase of vaccines covered under the Vaccines for Children program. The contracts’ period of performance is April 1, 2009 through March 31, 2010. Vaccines purchased under these contracts are to be used only in children 18 years of age and younger. Sale of the vaccine to any specific person or entity or reimbursement of vaccine costs is strictly prohibited. Free distribution of the vaccine is also prohibited except where the vaccine is administered in the context of Grantee immunization program activities. METHOD OF PAYMENT: Payment for orders utilizing Federal, State, or local funds will be made not later than 30 days after receipt of an invoice by the ordering office. You are further advised that the Prompt Payment Act, Public Law 97-177 (96 Stat. 85, 31 USC 1801) is applicable to payments under this contract and requires payment of interest on overdue payments and improperly taken discounts. Upon receipt and acceptance of partial deliveries, payment shall be made in accordance with the provision stated above. INSPECTION/ACCEPTANCE: Inspection/acceptance required by this contract will be completed no later than three working days after receipt of vaccine. OPTIONAL USE: Authorized users may place orders against this contract using Federal, State, and local funds. The Contractor shall honor all authorized optional funding orders from these agencies. PRICE REVISIONS: Each of these contracts includes a provision for the revision of unit prices at the option of the Contractors. Effective dates for the price revisions, if applicable, shall be as follows: August 1, 2009 and December 1, 2009. CDC will advise grantees of any such price revisions. DATA REPORTS TO MANUFACTURERS: The CDC shall submit monthly reports of vaccine data specifying doses by project, zip code, and NDC number in a standardized format. For all vaccines except direct ship vaccines, CDC will provide distribution data. Because CDC does not distribute direct ship vaccines, purchase data will be provided. Funding data will be provided at the grantee level and based on the grantees approved spend plan. The reports shall be submitted electronically to the Contractor by the 15 th of each month for the previous month. CDC shall also provide the Contractor with an annual Vaccine Ordering Forecasting Application (VOFA) spend plan for each grantee as well as an annual VFC providers list.
