REGISTRATION: Parents MUST come and sign a parental release form or send it in with their child on the day of the
clinic, registration begins at 9:30 am.
DROP OFF AND PICK-UP: Please have your child arrive no earlier than 30 minutes prior to the time of the clinic and
pick-up no later than 15 minutes after the clinic ends.

DEADLINE: Please detach and mail the lower portion of this form with your payment by Feb 5 . After this date
please bring the registration form and payment with you. If you miss the deadline, please phone and let us know you
will be attending. T-shirt for walk-ins placed on the day of clinic will not be received during clinic ( but @ a later pick-
up date). Please order the correct size needed Youth or Adult.
WALK-INS: You are welcome to attend! Please arrive early to fill out paperwork. Payment must be made at this
time.
SNACKS: Baked goods, fruit, drinks, and other snacks will be available for purchase. Items will be 25 cents. Pizza
will be available for purchase for $1.00 a slice.
PERSONAL ITEMS: Please label all personal items and keep them in a bag at camp. Please bring a water bottle for
drinks. Please keep in mind that children should dress appropriately for activities related to cheerleading. Tennis shoes
and socks are a must, hair should be worn up, and there should be no jewelry for safety reasons.

When you have any participant that is a minor, the parent or legal guardian should sign the name of the minor if the minor is not old enough
to sign the waiver themselves. Also, the parental consent portion should be signed by the parent or legal guardian.
                                       RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND
                                                                INDEMNITY AGREEMENT
                                                    TERRE HAUTE ALL-STAR CHEER UNIVERSITY
   In consideration of participating in the activities associated with Terre Haute All-Star Cheer University, Inc., I represent that I understand the
nature of such activities and that I am in good health and in proper physical condition to participate in the activities entered into with this group.
I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation
in the activity. I fully understand that this activity ( cheerleading and all things encompassed in this sport) involves risks of serious bodily injury,
including disability, paralysis, and death, which may be caused by my own actions, or inactions, those of others participating in the event or
activity, the conditions in which the event takes place, or the negligence of the “releasees” named below, and that there may be risks either not
known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, cost, and
damages I incur as a result of my participation in the activity which involves anyone connected with Terre Haute All-Star Cheer University, Inc.

I hereby release, discharge and covenant not to sue Terre Haute All-Star Cheer University, Inc. , its respective administrators, directors, agents,
officers, volunteers and employees, other participants, and sponsors, advertisers and if applicable, owners, lessors(RK Real Estate Group, LLC) of
premises on which the activity takes place (each considered one of the releasees herein) from all liability, claims, demands, loses, or damages
on my account caused or alleged to be caused in whole or in part by the negligence of the “releasees” or otherwise including negligent rescue
operations and future agree that if, despite this release, waiver of liability, and assumption of risk, I, or anyone on my behalf, makes a claim
against any of the “releasees”, I will indemnify, save, and hold harmless each of the “releasees” from any loss, liability, damage, or cost, which
may incur as the result of such claim.

I have read the Release and Waiver of Liability, Assumption of Risk and Indemnity Agreement, understand that I have given up substantial
rights by signing it and signed it freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional
release of all liability to the greatest extent allowed by law and agree that if any portion of this agreement is held to be invalid, the balance,
notwithstanding, shall continue in full force and effect.

PARENTAL CONSENT
And, I , the minor’s parent or legal guardian, understand the nature of the above referenced activities and the Minor’s experience and
capabilities and believe the minor to be qualified to participate in such activity. I hereby release, discharge, covenant not to sue and Agree to
Indemnify and Save and Hold Harmless each of the Releasees from all liability, claims, demands, losses or damages on the minors account
caused or alleged to have been caused in whole or in part by the negligence of the Releasees or otherwise including rescue operations, and
further agree that if, despite this release, I Will Indemnify, Save and Hold Harmless each of the Releasees from any litigation expenses,
attorney fees, loss liability, damage, or cost any Release may incur as the result of such claim.
