The Eagle Packet Consists of: 1. Eagle Scout Application • • • • • • 1994 or later printing of the application form is required. Read all instructions carefully. Print or type the information legibly. When entering all dates, be sure to enter month, day, and year (two digits each, 12/17/97 for example) List only the 21 merit badges that are required. For numbers 6 and 9, cross off those not earned. If any of those crossed off were earned, they can be entered in numbers 13 through 21, or later on an Eagle Palm application. Enter optional merit badges (numbers 13 to 21) in the order in which they were earned to satisfy Star, Life, and Eagle. Do not enter merit badges earned later on because they “look better." This can cause problems when the Star and Life dates are verified. 2. Six Letters of Recommendation • • • The names and addresses are supplied to the troop advancement chairman who sends out the letters and receives them back. They are then sent unopened to the District Advancement Chairman. One parent may be listed as a religious leader. If you have not held a regular job, list someone for whom you have done odd jobs. 3. Eagle Service Project Workbook • • Three copies all signed by the project supervisor, the Scoutmaster, the Committee Chairman, and the District Advancement Chairman. Make sure it is completely filled out. 4. Letter from Eagle Project Supervisor • This is a letter from the organization (church, school, nature center, etc.) that the project was done for. It should state that the project was carried out to their satisfaction. 5. Statement of ambitions and life purpose • This is a write-up (at least one page) of what you plan to do with your life. What interests you, what you might want to study, what profession you may wish to pursue, what hobbies you think will follow you, etc. 6. List of leadership positions, honors, and awards • This list (at least a half-page) should list all your leadership positions, honors, and awards both in and out of scouting. Church groups, academic organizations and awards, etc. should all be listed. 7. Service project final report • • • • • • This is a narrative description of the eagle service project that should show beyond question that you planned, developed, and gave LEADERSHIP to others. The report should be at least one full page, preferably longer. It should be typewritten with good grammar, spelling, and punctuation. Photos and drawings may be attached (and will be returned afterwards). Include a cover page titled MY EAGLE SCOUT SERVICE PROJECT and includes your name, the title of your project, your unit number, and date. What was the process you used in planning and developing the project? What was accomplished? How were things done? When? What plans that you had made did not work? Why? What changes in your operation did you then have to make? 8. Who worked on what, in what capacity, and how many hours did each spend? (give actual names of the workers, including adults or youth) 9. What materials, if any, were used? What were the costs involved? Who paid for these costs, or were they donated and by who? 10. What kings of things did you do for safety? What were your plans for transportation of possible injured workers to the hospital emergency room? 11. How did you demonstrate leadership? 12. What did you personally learn from this experience? 13. What recommendations do you have for other Eagle candidates? 14. What are your total hours from the planning stages through your final written Eagle Service Project Report?
