					BIOMEDICAL DEVICES CONCENTRATION
DIRECTIONS - IT IS RECOMMENDED THAT YOU PRINT OUT THIS PAGE.

This program planning template uses several worksheets and produces:
1. Schedule Information           2. Program of Study Form           3. Candidacy Application
The template is intended to be used for the standard situation where courses are taken only once.

Data is ONLY TO BE ENTERED on the Transcript Information form. The initial entry will be a plan
for your program through graduation.
For the initial data entry, use the following process:
          1 Enter the communication/academic history data
          2 Enter the following dates in Month-Year format. Check
             with your advisor if you are unsure of the information.
                     2.1 Date admitted to SJSU
                     2.2 Proposed date of graduation
                     2.3 Semester entering classified status
                     2.4 Date culminating experience (Project, Thesis or Exam) is completed.
          3 Course data are entered to represent your actual program. When this input is
             completed, the table should include the English Proficiency course, your prerequisite
             courses and 10 required core and elective courses.
                     3.1 For the ENGLISH PROFICIENCY COURSE (ENGR 200W or ENGR 100W),
                         enter the semester and year you took or plan to take the course, or received a waiver,
                         and the grade if you have completed the course. Enter W for waiver under
                         COMPLETE COURSE 1st TIME AT SJSU? And date waiver approved by GS&R.
                     3.2 For the PREREQUISTE COURSES these will be the courses that were
                         assigned in your notice of admission letter. REPLACE the information in the
                         template with the actual data for the course. If you have NO ASSIGNED
                         prerequistes, then DELETE (BUT DO NOT USE ROW DELETE) the data
                         in the course number, course title, and the units columns.
                     3.3 For the CORE AND ELECTIVE COURSES that are listed in the template, but
                         which NOT USED in your program of study DELETE (BUT DO NOT USE ROW DELETE)
                         the data in the course number, course title, and the units columns.
                     3.4 For each COURSE INCLUDED IN YOUR PROGRAM BUT NOT ON THE TEMPLATE,
                         you should have received an approval for a substitution from your advisor. Information
                         that needs to be entered for courses in this category includes the following:
                         3.4.1        DEPARTMENT offering the course
                         3.4.2        Course Number
                         3.4.3        Course Title
                         3.4.4        SJSU Semester Units or for a TRANSFER course, Alternate
                                      Institution EQUIVALENT SEMESTER Units (Other column)
                         3.4.5        SJSU course (Y) or TRANSFER course (N)
                         3.4.6        Letter Grade 1 (if course has been completed)
                         3.4.7        Term Grade 1 Obtained or Scheduled (Fall, Spring or Summer)
                         3.4.8        Year Grade 1 Obtained or Scheduled
                     3.5 Use the same technique for selection of the Culminating Experience.
                         3.5.1 If you are completing a THESIS, DELETE (BUT DO NOT USE ROW DELETE)
                         the ENGR 298 entry.
                         3.5.2 If you are completing a PROJECT, DELETE (BUT DO NOT USE ROW DELETE)
                         the ENGR 299 entries.
          4 Run the macro
                     4.1 Switch to the SCHEDULE worksheet
                     4.2 Enter ALT F8
                     4.3 Enter RUN
          5 Use of the Program Output
                     5.1 Print a copy of your Program of Study and submit to the MSE office for review and approval.
                         5.1.1 Once the Program of Study is approved, do not submit additional copies. If you have
                         changes, submit these through the MSE office using Changes in Program of Study forms.
                         5.1.2 Update the grades in the transcript section after each semester to confirm your current
                         grade status.
                     5.2 Print a copy of the Application for Candidacy after you have completed the required prereqisite
                         courses and English Proficiency course. Submit this to the MSE office for processing.
                         5.1.1 Once the Program of Study is approved, do not submit additional copies. If you have
                         changes, submit these through the MSE office using Changes in Program of Study forms.
                         5.1.2 Update the grades in the transcript section after each semester to confirm your current
                         grade status.
