Annual Report PARTNERSHIP “...developing the fullest potential...” December, 2004 As the Fulton County Partnership, Inc. begins our sixth year of operation, we would like to take this opportunity to inform our members as well as the community of the many ways we have been making a difference for families and individuals here in Fulton County. This Partnership Report, “…developing the fullest potential…” highlights just a few of the ways that our organization brings partners together to focus on Fulton County needs. That was our mission when we incorporated in 1999 and it remains our mission today. Although our mission has remained unchanged, our Partnership has continued to grow and to evolve from a consortium that began with one or two ideas into an organization that has, hopefully, grown to meet the many needs of those who define us and our collaboration. Recognizing that we are always a ‘work in progress,’ we continue to strive to make our system larger than the sum of its parts. In order to do that, however, we need the continued full support and participation of all of our board members, partners and our community. It is only with that support and participation that we can create the synergy of ideas that is so necessary to bring about change and to meet the challenges that lay ahead of us. There are some needs in Fulton County that are difficult to ignore and dental care and transportation are near the top of that list. The Partners have acted to address those needs. The Fulton County Partnership, Inc. Dental Clinic had over 700 patients by June 30, 2004. That is an increase of over 100 patients from this time last year. There are 12 dentists who volunteer their time and skill to help those Fulton County residents who do not have local access to dental care. Our dedicated staff not only works on teeth, but they work with patients to ensure that all of their dental needs are met. The Medical Assistance Transportation Program is meeting the medical transportation needs of over 440 Fulton County Access Card holders. MATP has provided over 1000 rides in 2004, and has reimbursed clients over $16,000.00 for mileage to medical appointments. Please continue to the next page for details about all of the FCPI Programs. Medical Assistance Transportation Program... MATP is designed to help keep county residents healthy. By removing transportation barriers, MATP hopes to encourage low income county residents with a valid Medicaid ACCESS Card to address their health needs before they become emergencies. The MATP program assists clients in overcoming transportation barriers in two ways: By providing door-to-door service to any medical appointment that is Medical Assistance compensable; or by reimbursing clients for their mileage to and from Medical Assistance compensable services. As of June 30, 2004, there are 483 Fulton County residents registered for this service. Employment Transportation Assistance Program... ETAP is a Welfare to Work Transportation Demonstration Program funded by the Pennsylvania Department of Transportation (Penn DOT). This program is aimed at helping low income working individuals overcome their transportation PROGRAMS barriers, and become self sufficient. Since July 1, 2001, the ETAP Program has helped more than 100 individuals become self sufficient. ETAP can help individuals with the following assistance – Transportation: up to twenty round trips to and from work which helps a client begin employment and earn one or two paychecks. Vehicle Repair: Up to $400 can be provided to get an existing vehicle in good operating condition for use in traveling to and from work. Vehicle Purchase: This service provides help to clients who have demonstrated a commitment to retaining employment by allowing them to purchase a vehicle with down payment assistance. Dental Clinic... The Fulton County Partnership, Inc. Dental Clinic has over 725 patients enrolled in the clinic. With an average of 2.36 visits per client, there have been more than 2,500 visits to the dental Clinic since it’s opening. Under the direction of a new Dental Clinic coordinator, the Clinic is becoming more and more involved in the broader community, by providing presentations for community groups, children, and classrooms. The Dental Clinic is staffed by a group of 12 volunteer dentists who are dedicated to meeting the needs of Fulton County residents. HelpLine... With the installation of a new phone system, the Partnership has been able to better quantify the number of calls to the HelpLine. What a surprise we had when we learned that the HelpLine was handling more than 100 calls a week. Most callers are asking for transportation solutions, however we see that the HelpLine is a growing resource to connect local people with local resources. We are pleased to be able to provide the HelpLine as a community service, and look forward to meeting more needs as time marches on. Health and Safety Expo... Held in conjunction with the American Cancer Society Relay for Life, the Health and Safety Expo was a huge success. It was a bleak and rainy day, but none the less, a good turn out braved the elements to support the Relay for Life and the Health and Safety Expo. This marked our second year moving the Expo from the school area to the Fulton County Fair PROGRAMS Grounds. There was a great representation of Human Service providers, and all those in attendance seemed to have a great time. We look forward to working with the American Cancer Society to build this into a grand county wide event! TRUCE (Teens Resisting Un-healthy Choices Everyday)... TRUCE is a program designed to recognize and reward those students who are consistently making positive decisions and acting upon them. We recognize that often teachers and administrators must focus their attention on the students that are making poor decisions, and the rest of the students may not get as much attention. TRUCE is designed to change that balance. TRUCE is also intended to be preventative in nature, keeping kids from starting drug use. The program is supportive, and never punitive. If we discover a child who has been using drugs, we strive to provide help, not punishment. More than 50 students enrolled in TRUCE in the second year of the program. We are looking forward to expanding enrollment our provider base in the upcoming year. In addition... The Fulton County Partnership in collaboration with the Fulton County Human Services Council has updated the Fulton County Human Services Directory. The 2004 edition of the Fulton County Human Services Directory represents the first major upgrade since 1996. This valuable resource is available to Fulton County residents at no cost through the Fulton County Partnership, Inc. From the beginning, the Fulton County Partnership has strived to do much more than just provide programmatic fixes for community needs. In 2004, the Partnership has brought more than 35 service providers to the table to continue the Collaboration Process. The Partnership is more than just an opportunity to meet and network, it provides a vessel by which organizations can come together and do real work collaboratively, sharing resources, and meeting community needs through an efficient and common sense approach. We look forward to expanding this in the coming year as we look for new ways to track outcomes for residents. We strive to help the community become a good steward of the resources available. PROGRAMS New partnerships were forged during the year as well. The Al’s Pals program which provides mentors to elementary school children has come under the umbrella of the Fulton County Partnership, as well as the Fulton County Safe Kids Coalition. The Partnership endeavors to provide services which help us meat the mandates of our mission statement in any way that we can. By providing support services for these programs, we hope to ensure the continued success of such valuable community initiatives. As we enter the new year, we are excited by the possibilities that exist. The Partnership has applied for funding to create a mobile health outreach center and is working with Tri-State Community Health Center to increase access to primary medical care services. The Partnership is looking at possibilities for expanding local transportation options as well. 2004 was a successful year for the Fulton County Partnership, and a great foundation was laid for the continued growth.
