                                                REINSTATEMENT AGREEMENT

Conditions for “Fast Track Back” Reinstatement:
The “Fast Track Back” option permits students to return to the university in fewer than twelve months after suspension.
To qualify for this option, a student must meet each of the following conditions:
 1. Have BYU cumulative GPA of at least 2.0 at time of suspension.
 2. Complete a minimum of twelve (12) credits of General Education/University Core credit following suspension.
     a) A CAC advisor must approve any courses for this option prior to enrollment in the courses.
     b) If a student has completed most or all of their General Education/University Core courses, approval may be granted to
          complete other solid academic courses.
     c) Courses may be completed through another college/university or through Independent Study.
     d) The GPA for the twelve credits must be at least 3.0 (“B” grade), with no repeats, W, UW, WE, I or IE type grades.
 3. Remain out of the university after suspension for a minimum of six (6) months.
When the student returns to the university after having fulfilled the above requirements, he/she will be readmitted on Previous
academic standing. However, if the student is ever suspended again, their standing will automatically become academic
Dismissal. During the first enrollment back after completing the FTB option, the student must complete a follow-up
appointment with an advisor from the Academic Support office as part of the reinstatement agreement.
Initiation of “Fast Track Back” Option:
To begin the process of this option, a student must have this form completed, including all signatures, within 60 days of the
academic suspension. The form is to be returned to the Academic Support Office, 2500 WSC. Final approval by an advisor
from Academic Support must be obtained prior to enrolling in any courses that will be used toward this option.
