        Committee (POC) submitted/referred to the Office of the Mayor?

        necessary examinations (written examinations, physical agility test,
        neuro-psychiatric/psychological examination, complete medical and
        physical examination, dental exam, final interview) for final
        evaluation?
    o   Does the Screening Committee prepare the Summary Table
        containing the list of the qualified applicants and the result of each
        examinations being conducted?
    o   Does the PNP Regional Screening Committee thru the PNP
        Regional Director submit list of qualified candidates to NAPOLCOM
        Regional Director for certification?

   NATIONAL HEADQUARTERS/NATIONAL SUPPORT UNITS LEVEL

    o   Is the allocation for recruitment quota approved by NAPOLCOM?
    o   Is the Recruitment Notice posted in at least three (3) conspicuous
        public places and advertised through trimedia or other forms of
        information campaign?
    o   Is the Screening Committee organized in accordance with MC
        2007-009?
    o   Does the Summary Report on the results of each examination
        submitted to NAPOLCOM contain the following data?
         Written qualifying examination
         Physical Agility Test (PAT)
         Medical and Physical Examination (MPE)
