       Springfield, Illinois – Robert E. Hewson, Jr., Acting Director of the Illinois Department
of Professional Regulation, announced the following disciplinary actions taken by the
Department during the month of May 2003.


          ARCHITECTS, LAND SURVEYORS, PROFESSIONAL ENGINEERS
                       AND STRUCTURAL ENGINEERS

       Electronic Architects, Mount Vernon; Kenneth B. Nelson, Mount Vernon; and, Van
C. Bushue, Mount Vernon – ordered to cease and desist using the term “Architect” and from
engaging in the unlicensed practice of professional engineering.

         Edward C. Kerr, Palatine, and Kerr & Associates, Palatine – ordered to cease and
desist from engaging in the unlicensed practice of professional engineering.

        John Lupinos, Chicago – ordered to cease and desist the unlicensed practice of
architecture.

       Marsurco Inc., Frankfort; Town and Country Surveying, Frankfort; and Marcella
Marcus, Tinley Park and Wilmington – ordered to cease and desist from engaging in the
unlicensed practice of land surveying.

        Pablo Morales, Chicago – ordered to cease and desist from engaging in the unlicensed
practice of architecture, structural engineering and professional engineering.


                                   ATHLETIC TRAINING

       Heather N. Goss, Skokie – athletic trainer license (096-001784) issued with reprimand
and fined $200 for practicing without a license for six months.

       Brian Edward Studnicki, Pontiac – athletic trainer license (096-001785) issued with
reprimand and fined $570 for practicing without a license for two years.


         BARBER, COSMETOLOGY, ESTHETICS AND NAIL TECHNOLOGY

       Elijah Bell, Chicago – cosmetologist license (011-263108) issued and placed on
probation for four years due to criminal conviction history.

         Corey Blackledge, Schaumburg – cosmetologist license (011-263107) issued and
placed on probation for two years after he was discharged from the U.S. Army for involve ment
in distribution of controlled substances.

        Ketina L. Gibson a/k/a Ketina White, Chicago and Richton Park – cosmetologist
license (011-219342) indefinitely suspended for failing to file Illinois income tax returns for
1996, and 1998 to 2000, and failing to pay Illinois income taxes for 1997.

       Tracey Kurta, Waukegan – cosmetologist license (011-263044) issued and placed on
probation for one year and fined $1,250 for practicing without a license since 1994.

       Martin W. McKune, Chicago – cosmetologist license (011-209741) reprimanded and
fined $800 for practicing on a non-renewed license.

       Terrell Owens, Aurora – ordered to cease and desist the unlicensed practice as a barber
or cosmetologist.

        Maria Quansah, Chicago, and Africana Hair Braiding, Chicago – Quansah is ordered
to cease and desist offering cosmetology or barbering services, and Africana Hair Braiding is
ordered to cease and desist offering cosmetology services.

        Shondal Reid, Crest Hill – barber license (006-062328) issued and placed on probation
for one year due to criminal conviction history.

       Carolyn Sue Taylor, Virginia – cosmetologist license (011-148073) indefinitely
suspended for practicing on a non-renewed license.

       John Tomek, Elmwood Park – barber license (006-056605) reprimanded and fined
$150 for allegedly aiding and abetting the unlicensed practice of nail technology for three
months.


The following individuals’ barber licenses were placed in refuse to renew status after defaulting

suspended after defaulting on an Illinois Student Assistance loan.

       Karen Nash, Chicago – dentist (019-019941) and controlled substance licenses restored
and placed on indefinite probation.

         Evelyn Richardson Wiley, Wheaton – dentist license (019-014774) reprimanded for
allegedly failing to properly maintain patient records concerning at least three patients and
failing to take radiographs of sufficient quality as to be useful for diagnostic purposes for at least
three patients.


                      DETECTIVE, ALARM, SECURITY AND LOCKSMITH

        Ralph W. Briscoe, McHenry – private detective license (115-001160) reprimanded and
fined $500 for allegedly engaging in the practice of a private security contractor agency without
a license, including employing unlicensed individuals to provide private security guard service.

        Patrick O. Donaldson, Chicago – permanent employee registration card (129-023421)
placed in refuse to renew status for being more than 30 days delinquent in the payment of child
support.

        Edwards Company, Inc., Lombard – private alarm contractor agency license (127-
000446) reprimanded and fined $3,000 for allegedly employing three individuals whose
applications were not submitted in a timely manner, employing two individuals who did not have
active permanent employee registration cards or pending applications, and employing one
individual whose permanent employee registration card was non-renewed.

for allegedly failing to follow- up on a chest X-ray that had been ordered by another physician.

       Kyuil Lee, Rock Island – physician and surgeon license (036-053074) indefinitely
suspended after violating the terms and conditions of a previously ordered probation.

         Tanin Parich, Alton – physician and surgeon license (036-050320) reprimanded for
refilling a kidney transplant patient’s medications without consulting with the patient’s
nephrologists.

       Clifford A. Poppens, Owensboro, KY – physician and surgeon license (036-053586)
placed on probation through Feb. 28, 2007, after being disciplined in Kentucky.

        Bonafacio M. Rivera, Northbrook – physician and surgeon license (036-052549)
indefinitely suspended for failing to file and pay Illinois income taxes.

indefinitely suspensed after being disciplined in Missouri.

       Nick Goodwin, Toledo – licensed practical nurse license (043-086914) restored and
placed on probation for two years.

        Cynthia A. Hughes, Lake in the Hills – registered nurse license (041-227167) placed on
indefinite probation for writing an unauthorized prescription for herself.

         Starlene Jordan, Belleville – registered nurse license (041-272096) placed on probation
for three years for diverting Darvocet for self- use from St. Elizabeth’s Hospital in Belleville and
after testing positive for propoxyphene.

        Jennifer M. Lowe, Decatur – licensed practical nurse license (043-087416) placed on
indefinite probation after defaulting on an Illinois Student Assistance loan.

       Ellen J. Miller, Fox Lake – registered nurse license (041-139489) restored and placed
on indefinite probation.

       Valentina Nortridge, Freeport – registered nurse license (041-222213) indefinitely
suspended for failing to file and pay Illinois income taxes.

       Eleafious Taylor Jr., Chicago – licensed practical nurse license (043-034707) restored
and placed on indefinite probation.


                                          OPTOMETRY

       Daniel W. Sherwood, Galesburg – optometrist license (046-006645) reprimanded and
fined $1,500 for failing to properly document a patient’s chart.


                                           PHARMACY

        Courtney Bartow, Yorkville – pharmacy technician license (license not yet issued)
issued and placed on probation for one year due to her arrest and conviction of retail theft.

                                 PROFESSIONAL BOXING

       Rick Gravesen, Geneseo; Cenobite Productions, Geneseo; LT’s Bar & Grill,
Rockford; and Todd Gage, Rockford – ordered to cease and desist from promoting, offering or
holding any unregistered boxing events and any event which constitutes ultimate fighting.


                                        PSYCHOLOGY

        Dennis P. Selvig, Chicago – clinical psychologist license (071-004066) reprimanded for
allegedly engaging in unprofessional and unethical conduct by failing to maintain his objectivity
in the assessment of clients in his performance of fitness- for-duty evaluations.

       Thomas C. Todd, Evanston – clinical psychologist license (071-003520) placed on
probation for one year and fined $1,000 after allegedly engaging in unprofessional conduct.


                                ROOFING CONTRACTORS

       Spaulding Construction, Decatur – roofing contractor license (104-004201)
reprimanded and fined $150 for practicing on a non-renewed license.


                                       SOCIAL WORK

        James P. Gorski, Lindenhurst – clinical social worker license (149-002278)
indefinitely suspended for engaging in unprofessional conduct by downloading numerous
pornographic pictures using his office computer while he was Director of Youth and Family
Services at a mental health facility.


                                 VETERINARY MEDICINE

       Guardial S. Basran, Chicago – veterinarian license (090-004575) reprimanded and
fined $500 for failing to perform a thorough examination of a dog, which led to a misdiagnosis
regarding the recommended care and treatment of the dog.

       Laura L. Portee, Harrisburg – veterinarian license (090-007353) reprimanded and
fined $200 for practicing on a non-renewed license.
